超声心动图对房间隔缺损封堵术后右心功能及形态的评价

摘　　要：目的:探讨超声心动图对房间隔缺损(ASD)封堵术后右心功能及形态变化的评价。方法:73例接受国产封堵器封堵术的继发孔型ASD患者,于术前1d、术后3d及3个月分别进行经胸超声心动图(TTE)检查,测量右室前后径、右房横径和长径,肺动脉压力,右室侧壁基底部组织收缩期运动速度(S′)、舒张早期运动速度(E′)及舒张晚期运动速度(A′)。结果:本组患者ASD大小为8～33mm,封堵伞直径为10～40mm;术后3d66例右室前后径、右房横径和长径较前缩小,3个月70例内径恢复在正常范围。结论:ASD封堵术在消除异常分流的同时,可有效的改善右心心功能及几何形态,降低肺动脉压力。Objective： To evaluate right cardiac function and morphology in patients after transcatheter closure of atrial septal defect （ASD） by echocardiography. Methods： In 73 patients with transcatheter closure of ASD with domesticmade occluder device , echocardiographic examinations were resp6ctively made 1 day before , 2 days and 3 months after the closure , to calculate the size of anteriorposterior diameter of right ventricle , the size of transverse diameter and long diameter of right atrium,the pulmonary artery pressure ,the systolic movement velocities （S＇）,early diastolic movement velocities （E＇） and late diastolic movement velocities （A＇） of basic lateral walls of right ventricle. Results： ASD size measured by echocardiography was 8~ 33mm and size of used occluder was 10- 40mm. 3 days after transcatheter closure, 66 patients anterior-posterior diameter of right ventricle , the size of transverse diameter and long diameter of right atrium,the pulmonary artery pressure,S＇ ,E＇, A＇ were significantly lessened. 3 months after transcatheter closure, 70 patients right cardiac size became normal. Conclusion： Transcatheter closure of ASD can improved markedly right cardiac function and geometric, reduce pulmonary artery pressure and at the same time eliminate the abnomal shunt.
