胃肠安方抑制胃癌转移的临床观察及对胃癌细胞株侵袭转移的影响

摘　　要：目的:晚期胃癌多存在脾虚为本,邪实为标的病机特点,胃肠安方是以健脾为主,辅以清热解毒、软坚化痰的治疗胃癌的临床经验方剂。本研究旨在观察胃肠安方对胃癌转移的抑制作用。方法:对148例胃癌术后病例进行随机分组对照研究。并用transwell小室法检测胃肠安方对MGC-803细胞株侵袭能力和趋化运动能力的影响。结果:胃肠安方组及中西医结合组胃癌术后1、2、3年生存率明显高于化疗组,差异有显著性(P<0.05)。胃癌术后1年转移率,胃肠安组与化疗组,中西医组与化疗组比较,差异均有显著性(P<0.05),胃肠安组与中西医组比较,差异无显著性(P>0.05);10%胃肠安方药物血清处理MGC-803细胞株24h,能明显抑制MGC-803细胞株的侵袭、趋化运动能力。结论:胃肠安方对胃癌术后转移可能有一定的预防作用。胃肠安方能明显抑制MGC-803细胞株的侵袭、趋化运动能力。Objective：To observe the effect of Weichang＇an （WCA, a Chinese traditional medicine） in preventing post-operative metastasis of progressive stage of gastric cancer. Methods： A prospective randomized, controlled study was conducted by dividing the148 patients of progressive staged gastric cancer after radical operation into the WCA group, the chemotherapy（CT） group and the WCA＋CT group, to observe the survival rate, metastasis rate. Transwell chamber assay was performed to determine the effect on invasion and migration capacities of MGC-803 cells.Results：The1,2 and 3 year survival rates after operation in the WCA＋CT group were 89.51%,69.77% and 55.76 % respectively, which was significantly higher than that in the CT group （83.86%,59.33% and 49.43%） （P〈0.05）,but showed no significant difference as compared with that in the WCA group （93.23%,79.34% and 71.78%）. Only 1 year metastasis in the WCA group was 15.25%, and in the WCA＋CT group was 15.52%, which were significantly lower than that in the CT group （35.48%, P〈0.05）. But the comparison of 2 year metastasis rate among the 3 groups （28.81%, 41.38% and 45.16%） and 3 year metastasis rate among the 3 groups （33.90%, 46.55% and 51.61%） were not insignificantly different. After 24h treatment of 10%WCA drug serum,inhibitory rate of invasion and migration capacities of MGC-803 cells were significantly lower than normal rat serum control group. Conclusion： WCA has preventive effect on relapse and metastasis in post-operative gastric cancer patients. WCA can inhibit migration,invasion of MGC-803 cells.
