小儿腹腔镜在小儿肾切除术中的应用(附6例报告)

摘　　要：目的探讨腹腔镜手术在小儿肾输尿管切除术中的应用。方法对2000年1月至2006年12月间的6例肾发育不全和重复肾合并输尿管异位开口的患儿使用小儿腹腔镜手术进行肾输尿管切除术或部分肾及输尿管切除术。结果6例手术均获得成功,手术耗时110～253min,平均149min,术中出血15～20mL,平均15mL,术中出现腹膜前气肿1例、膀胱损伤1例,未出现穿刺孔感染、出血、麻醉意外等并发症。术后随访4～6个月,平均5.2月,恢复良好。结论小儿腹腔镜能够安全有效地应用于肾或部分肾输尿管切除术,有利于病孩的术后恢复。小儿腹腔镜手术中较易发生各种并发症,熟练的常规手术经验是腹腔镜手术最重要的安全保障。Objective To study the clinical application of laparoscope in children＇s nephrectomy. Methods A retrospective analysis was made on clinical reports of 6 cases of children laparoscopic operations from January 2000 to December 2006. These children suffered from anephrogenesis or renal duplication incorporate with ectopic ureterostoma and underwent laparoscopic operations. Results Six patients received nephro-ureterectomy or ureterectomy, and 2 cases of complication occurred during the first 2 operations, which included 1 case of preperitoneal emphysema and 1 case of bladder injury. No anesthetic accident,infection and haemorrhage on paracentesis point occurred. All tire patients were followed up for 4 -6 months, and their recovery was really good. Conclusion Laparoscope can be used in the children＇s nephro-ureterectomy or ureterectomy safely and effectively, which benefits patients＇recovery. Good routine operation skills is the most important guarantee of safe laparoscopic operations.
