可膨胀自锁式髓内钉在四肢长骨骨折中的应用

摘　　要：目的探讨可膨胀自锁式髓内钉治疗四肢长骨骨折的方法和临床效果。方法对68例四肢长骨骨折患者行闭合可膨胀自锁式髓内钉治疗。对骨折愈合情况以及患肢的功能进行临床疗效评价。结果患者伤口均一期愈合。68例均获得随访，时间12～16个月，67例获骨性愈合，愈合时间平均为4．8个月。1例股骨远端骨折逆行钉出现骨不连，经二次植骨钢板固定后获骨性愈合。骨折愈合及关节功能Johner-Wruh评分：优35例，良24例，可7例，差2例，优良率为86．8％。结论可膨胀自锁式髓内钉能简化手术操作，具有损伤小、感染率低、骨折愈合率高等优点，是治疗四肢长骨骨折较好的方法。Objective To study the technique and the clinical effect of treating long bone fracture of extremities by self-locklng expandable intramedullary nail system. Methods 68 cases with long bone fracture of extremities were treated by self-locking expandable intramedullary nail system. The clinical effect was evaluated postoperatively. Results The follow-up time was 12 - 16 months for 68 cases, and the recovery time was 4. 8 months. Nonunion of the distal femoral fracture was found in 1 case by retrograde nailing, The second operation had been done by plate and bone graft and bony union was obtained. Johner-Wruh score ： excellent in 35 cases, good in 24 cases, fair in 7 cases, and poor in 2 cases. The rate of excellent and good results was 86. 8%. Conclusions The treatment of long bone fracture of extremities by self-locking expandable intramedullary nail system brings less damage, low infection rate, better fracture healing. It is a considerable method for treating long bone fracture of extremities.
