摘 要:目的:建立反相高效液相色谱紫外检测测定血清中异丙酚浓度的方法,并观察病人手术血清中异丙酚浓度与意识水平的关系。方法:用乙腈高氯酸(2∶1)沉淀血清蛋白后取上清液,以邻苯二甲酸二丁酯作为内标定量。结果:本法测定的线性范围为0.5μg·ml-1~4.0μg·ml-1,最低检测限为0.1μg·ml-1。血药浓度的测定说明:异丙酚无蓄积作用,维持意识消失的血药浓度为2.1μg·ml-1以上,术后苏醒时的血药浓度约为1.0μg·ml-1。结论:本法简单、快速,适用于手术过程中异丙酚药动学的监测。OBJECTIVE: To develop a method for the determination of serum propofol by using a reverse phase HPLC and UV detector system, and to study the relationship between serum propofol concentration and unconsciousness in patients during surgical operation. METHODS: Propofol was extracted from serum by acetonitrile/perchloric acid (2∶1) precipitation and centrifugation. Dibutyl phthalate was used as the internal standard. RESULTS: The linear range and the lower limit for propofol quantification were within 0.5μg·ml -1 ~4.0 μg·ml -1 and above 0.10 μg·ml -1 , respectively. Our study suggests that the serum propofol concentration appropriate to the maintenance of unconsciousness should be more than 2.10 μg·ml -1 , and the concentration at which the patients can be awakened is approximately 1.0 μg·ml -1 . Our results also demonstrate that propofol does not accumulate in the body although it is administered continuously. CONCLUSION: The HPLC method for the measurement of serum propofol is simple and quick. It may be useful for the pharmacokinetic study during surgical operation.
