摘 要:目的:研究糖克煎剂对2型糖尿病胰岛素抵抗模型大鼠肿瘤坏死因子-α(TNF-α)及葡萄糖转运体4(GLUT4)在组织中表达的影响,探讨糖克煎剂对胰岛素抵抗的作用机制。方法:用链脲佐菌素(STZ)加高热量饲料的方法复制出大鼠模型,分别用中药复方糖克煎剂和罗格列酮作用于模型大鼠;用胰岛素敏感指数(ISI)衡量胰岛素抵抗程度;用RT-PCR法检测脂肪组织中TNF-αmRNA的表达水平;用免疫组化ABC法观测骨骼肌组织中GLUT4的表达水平,并用电镜观察其在骨骼肌细胞中的分布情况。结果:模型组ISI明显低于正常对照组(P<0.05),治疗组ISI与模型组相比均明显提高(P<0.05);模型组脂肪组织中TNF-αmRNA的表达明显高于正常对照组和治疗组(P<0.01);免疫组化发现骨骼肌组织中GLUT4的蛋白表达各组间差异无显著性,但电镜观察显示正常对照组的GLUT4阳性颗粒多靠近骨骼肌细胞膜边缘,而模型组的GLUT4阳性颗粒靠近骨骼肌细胞膜边缘者则明显减少。结论:糖克煎剂可能通过减少TNF-α和增加GLUT4的转位而非影响其蛋白水平而改善胰岛素抵抗。Objective: To study the effect of Tang Ke Decoction on tumor necrosis factor α(TNFα) and glucose transporter 4 (GLUT4) expression in tissues of model rats of insulin resistance of type two diabetes mellitus, and to investigate the mechanism of the function of Tang Ke Decoction to insulin resistance. Methods: Rat model of insulin resistance of type two diabetes mellitus was duplicated by using strepozotocin (STZ) and high caloric diet, and using Tang Ke Decoction of compound Chinese herbs and rosiglitazone acted on model rats individually. The degree of insulin resistance was measured by insulin sensitivity index (ISI). The expression of TNFa mRNA was assayed in adipose tissue by RT-PCR. GLUT4 protein expression was assayed by immunohistochemical method in skeletal muscule tissue,and its distribution in skeletal muscle cell was observed with electron microscope. Results: ISI of model group was significantly lower than that of control group( P 〈 0.05), ISI of theraputic groups were significantly increased than that of model group( P 〈 0.05) ; The expression of TNFa mRNA in adipose tissue of model group was significantly higher than that of control group and theraputic groups( P 〈 0.01). Immunohistochemistry revealed that there was no significant difference of GLUT4 protein expression in skeletal muscle tissue among variant groups; however electron microscope observed that positive granules of GLUT4 were mostly located in muscle cellular membrane in control group, but mostly in cytoplasm in model group. Conclusion: Tang Ke Decoction increased the insulin sensitivity by decreasing TNFa and increasing the transposition of GLUT4 probably, but not by decreasing the level of GLUT4 protein.
