垂体后叶素在腹腔镜下子宫肌壁间肌瘤剔除术中的应用

摘　　要：目的探讨垂体后叶素在腹腔镜下子宫肌壁间肌瘤剔除术中的止血效果。方法将腹腔镜下子宫肌壁间肌瘤剔除术患者70例随机分为2组,A组予术中子宫局部注射经生理盐水20 mL稀释的垂体后叶素12 IU;B组术中同样方法仅注射缩宫素20 IU。测定术中出血量、术中血压变化、手术时间、手术后血红蛋白变化、术后肛门排气时间及术后住院时间。结果所有患者均顺利完成手术,无中转开腹,术中无并发症发生。A组术中平均出血量少,平均手术时间短,术后第1天血红蛋白下降少,与B组比较均有显著性差异(P均<0.01)。用药后15 min A组收缩压上升比B组明显,用药后30 min及60 min收缩压变化无显著性差异。2组术后肛门排气时间、术后并发症、术后住院时间均无显著性差异。结论在腹腔镜下子宫肌壁间肌瘤剔除术中,垂体后叶素局部注射止血效果优于缩宫素。Objective It is to approach the hemostasis effect of pituitrin during laparoscopic myomectomy for the patients with intramural hysteromyoma. Methods 70 patients with intramural hysteromyoma received laparoscopic myomectomy were divided into two groups randomly. In group A, 12 IU pituitrin with NS 20 mL were injected into uterus while in group B 20 IU oxytocin instead of pituitrin were given in the same way during the operation. The blood loss volume and systolic blood pressure changes during operation, and the operative time, hemoglobin changes and complications after operation were observed. Results All the operations were clone successfully, without severe complications happened. Mean bleeding volume and mean operative time were significant difference between the two groups （P 〈 0.01）. The first day after operation, hemoglobin deceased also significant difference between them. But there was no significant increasing of blood pressure after 30 and 60 minutes in both groups. There were no significant differences in anus gas discharge, side effect and in-hospital time after operation in both groups. Conclusion Pituirrin by local injection into uterus during laparoscopic myomectomy for intramural hysteromyoma is more effective than oxyrocin.
