连续硬膜外腔神经阻滞在会阴手术中的应用

摘　　要：目的观察连续低位硬膜外麻醉在会阴、肛周手术中的作用效果。方法选择80例ASAⅠ￣Ⅲ级的患者,在L3￣4或L4￣5椎间隙行硬膜外穿刺和硬膜外麻醉管理,药物给予盐酸利多卡因或盐酸氯普鲁卡因,试验量3￣5ml,初次量总和在8￣15ml,术中监测生命体征。结果麻醉穿刺一次性成功率达100%,麻醉效果良好,生命体征趋于稳定,生理干扰小,并发症少。兼能实施术后硬膜外连续镇痛,可减轻患者的疼痛和应激反应。结论连续低位硬膜外腔麻醉可常规应用于会阴、肛周手术。Objective to observe the effect of continuity epidural anesthesia on perineum or Crissum stugery. Methods 80 cases of ASA Ⅰ - Ⅲ were undertaken epidural puncture and anesthetic management in the intervertebral of lumbar 3-4 or lumbar 4-5. A test dose of Lidocaine Hydrochloride or Procaine Hydrochloride 3-5 ml and 8-20 ml total dose were administered . The vital signs were morhitoring during the operation.Results The achievement ratio of narcotic puncture by first time was 100 percent and vital signs of the patients keep stable,with little infer ference and few complications. Postoperation epidural analgestie released the pain and stress reaction.Conclusions Continuity epidural anesthesia can be applied to perineum or crissum surgery.
