摘　　要：目的探讨2型糖尿病(T2DM)患者糖耐量正常的一级亲属胰岛素(Ins)第一时相分泌的变化及其与T2DM发病机制的关系。方法选无DM家族史糖耐量正常的健康人35名为对照组[NGT(-)]组,已确诊的T2DM患者糖耐量正常的一级亲属33名为研究[NGT(+)]组。放射免疫分析法测定两组FIns及静脉葡萄糖耐量试验(VGTT)后2min、4min、6min、8min血清Ins含量。结果(1)NGT(+)组的腰围、BMI、TG与NGT(-)组相比差异有统计学意义。(2)NGT(+)组2min处Ins低于NGT(-)组,两组间差异有显著统计学意义。(3)NGT(+)组2min处Ins值与腰围呈负相关。结论T2DM患者一级亲属在糖耐量正常时已出现Ins第一时相分泌下降,且与腰围呈负相关,可作为向IGT和DM发展的早期预测指标。Objective To investigate the changes of the first-phase insulin secretion in NGT first-degree relatives of T2DM patients , Methods The NGT subjects without diabetes family history [NGT（-） group,n= 35] and the NGT first-degree relatives of diagnosed diabetes patients [NGT（＋） group,n= 33] were selected. The levels of fasting serum insulin and 2-min, 4-min, 6-min, 8-min serum insulin after intravenous glucose tolerance test（IVGTT） were determined Results （1） NGT（＋） group showed higher levels of WC,BMI,and TG than in NGT（-）group. （2）The insulin level at 2 min of NGT（＋） group was lower than that of NGT（-） group[（4.38±0.53 ）mIU/L vs （4.68±0.39） mIU/L,P〈0.05], （3）The insulin level at 2 min of NGT（＋） group was correlated with waist circumference. Conclusion The NGT first-degree relatives of T2DM have the declined first-phase insulin secretion （FPIS）, and FPIS is negatively correlated with waist circumference.
