摘  要:【目的】了解服用丙戊酸治疗的癫痫患儿的体质指数(body mass index,BMI)与血浆胰岛素、瘦素、肉毒碱、丙戊酸的剂量、疗程、质量浓度的关系。【方法】测定32例服用丙戊酸的癫痫患儿和33例对照儿血浆胰岛素、瘦素、游离肉毒碱浓度并计算胰岛素抵抗值、BMI,并对癫痫患儿的BMI与胰岛素抵抗值、瘦素、肉毒碱水平、丙戊酸的剂量、疗程、质量浓度进行相关分析。【结果】丙戊酸治疗组BMI、胰岛素、胰岛素抵抗值、瘦素水平高于对照组,游离肉毒碱水平低于对照组。BMI与胰岛素抵抗值、瘦素水平显著正相关,与肉毒碱浓度、VPA剂量、疗程、质量浓度无相关性。【结论】丙戊酸治疗可引起肥胖程度增加、胰岛素抵抗、瘦素水平增加及肉毒碱减少。BMI与胰岛素抵抗值、瘦素水平正相关,与肉毒碱水平、VPA剂量、疗程、质量浓度无相关性。[Objective] To investigate the correlations between body mass index (BMI) and plasma insulin, leptin levels, carnitine levels,dosage and plasm concentration of valproate when epileptic children were treated with valproate. [Methods] Plasma insulin, leptin, free carnitine levels were measured in a group of 32 epileptic children treated with valproate and 33 age-matched children as control group. Insulin resistance(Homa-IR) and BMI were calculated. The associations between BMI and insulin resistance, leptin levels, carnitine levels, dosage and plasm concentration of valproate were analyzed. [Results] BMI, insulin, insulin resistance and leptin levels were significantly higher, but free carnitine levels were significantly lower in the valproate-treated group than those in the control group. In valproate-treated group, BMI were positively correlated with insulin resistance and leptin levels, but exhibited no correlations with carnitine levels, dosage or plasm concentration of valproate. [Conclusions] Valproate therapy could lead to weight gain, insulin resistance, leptin resistance and carnitine deficiency. BMI are positively correlated with insulin resistance and leptin levels, but not related to carnitine levels, dosage or plasm concentration of valproate.
