三乙醇胺乳膏防治鼻咽癌颈部急性放射性皮炎的临床疗效观察和护理体会

摘　　要：目的观察三乙醇胺乳膏在预防和治疗鼻咽癌患者急性放射性皮炎(简称RD)中的作用。方法58例接受放疗的鼻咽癌患者随机分为两组,其中32例在放疗开始时预防性使用三乙醇胺作为预防组,26例在出现放射性皮炎时开始用药作为对照组。结果预防组和对照组放射性皮炎发生率均为100%,但防治组与对照组相比,皮肤损伤程度相对较轻,导致相同程度RD的射线累积剂量相对较高,RD的平均治愈时间明显缩短,三方面的差异均有显著性(P<0.05)。结论预防性使用三乙醇胺乳膏具有减轻皮肤放射性损伤,提高皮肤对射线的耐受剂量及加速急性放射性皮炎创面愈合的作用。Objective：To evaluate the effect of Trolamine cream for preventing and treating acute radioepidermitis in patients with nasopharyngeal carcinoma. Methods：58 patients with nasopharyngeal carcinoma, undergoing radiotherapy, were randomized into 2 groups：Trolamine cream was used in 32 eases for prevention and treatment of acute radioepidermitis as radiotherapy was carded out ,while Trolaminein cream was used in 26 cases in the control group as grade I radioepidermitis appeared. Results：The rate of radioepidermitis was 100% in two groups respectively,P〈0.05. The degree of skin destruction in the prevention group was lower than that in the control group with statistically significant difference （P〈0.05）. The accumulative dose of ray induced same grade radioepidermitis in the prevention group was higher than thatin the control group,P〈0.05. The mean time for curing radioepidermitis in the prevention group was （9.2 ± 2.31 ） days as compared with （ 14. 4 ± 3.35 ） days of the control group with statistically significant difference （P〈0.05）. Conclusion ： Prophylactic application of Trolamine cream can postpone the development of radioepidermifis, raise the tolerance dose of radiation in the skin, and hasten healing of acute radiation dermatitis.
