中西医结合治疗创伤感染性小腿骨皮缺损267例临床观察

摘　　要：目的:探讨中西医结合早期修复治疗创伤感染性小腿骨皮缺损的临床疗效。方法:1982年1月～2004年12月间,应用中医中药内外兼治与一期完成病灶彻底清除和骨皮缺损修复的手术方法,治疗小腿创伤感染性骨皮缺损267例。结果:手术成功264例,成功率为98.9%;因受区血管条件差,术后发生血管危象探查无效骨皮瓣坏死3例;依照第十四届全国中西医结合骨伤科学术研讨会提出的疗效评价标准:优166例,良97例,可1例,差3例,优良率为98.5%。结论:应用中医药内外兼治并结合显微外科技术一期完成病灶清除和骨皮缺损修复的方法,是早期治疗创伤感染性小腿骨皮缺损的有效方法。Objective：To probe the clinical effect of integrated traditional and western medicine on the leg osteocutaneous defect due to wound infection in the early stage. Methods, From January 1982 to December 2004, traditional Chinese medicine for both internal and external treatment was adopted together with the first stage operation for removing focus and repairing defect for 267 patients who suffered from leg osteo-cutaneous defect due to wound infection. Results：The operation succeeded in 264 patients, the successful rate was 98.9 %. 3 patients had skin flap thanatosis because of blood vessel crisis. According to the criteria of effect evaluation given by the 14th National Seminar on Orthopedics and Traumatology of Integrated Traditional and Western Medicine, 166 cases were excellent, 97 cases good, 1 case OK, 3 cases bad,the excellent-good rate was 98.5 %. Conclusion：The application of traditional Chinese medicine for both internal and external treatment together with microsurgery technique in the first stage operation for removing focus and repairing defect is an effective treatment for leg osteocutaneous defect due to wound infection in the early stage.
