重症急性胰腺炎的手术治疗

摘　　要：本文对近８年来９１例重症急性胰腺炎病例进行了回顾性研究，根据ＣＴ影像或术中所见胰腺坏死的程度将病例分为轻度，中度和重度。本组资料提示：轻度和中度病例宜以保守治疗为主，重度病例应以手术治疗为主。手术病人术前需纠正休克或成人呼吸窘迫综合征，术中应充分游离胰腺，十二指肠造瘘对阻止胰腺炎的发展和防止胰腺或胰周脓肿的形成有重要作用。This paper reports the results of 91 patients with severe acute pancreatitis (SAP) in our hospital from 1988 to 1995. The results suggest that the patients with light or middle SAP should mainly receive conservative treatment and those with severe SAP should undergo operations. To dissect the pancreas as fully as possibe is the key of the surgical intervention. Duodenostomy plays an important role in prevention of development of pancreatitis and formation of pancreatic abscess.
