摘  要:目的观察生脉注射液对心搏骤停患者复律后外周血中性粒细胞核转录因子-kappaB(nuclear factor-kappaB,NF-κB)的活化程度、肿瘤坏死因子-a(tumor necrosis factor-alpha,TNF-a)水平及复苏预后的影响。方法分别用酶免疫组织化学技术和放射免疫法检测外周血中性粒细胞NF-κB的活化程度和血清TNF-a水平,并观察患者生命体征和实验室检查指标的变化。结果①实验组生命体征恢复时间、程度均优于对照组(P<0.05);②对照组NF-κB核移位程度、TNF-a水平高于实验组(P<0.05);③生脉注射液呈剂量依赖性地抑制中性粒细胞NF-κB的活性、血清TNF-a水平(P<0.05);④NF-κB活性、TNF-a水平同患者生命体征恢复情况呈负相关,生命体征恢复的越好,NF-κB活性及TNF-a水平越低。结论在心搏骤停复律后联合应用生脉注射液和常规抢救药物,确有维护心脏和循环功能、提高心肺脑复苏预后的良好疗效,其作用机理可能是抑制NF-κB信号传导通路及其调控的炎症因子。Objective To observe the effects of Shengmai Injection on NF-κB activation of the neutrophile granulocyte, serum TNF-a level and the prognosis of resuscitation on patients who had recovered from cardiac arrest. Methods The enzyme labeled immunohistochemical method was used to determine the NF-κB activation degree. Radio-immunity (RI) was used to determine the blood serum level of TNF-a. Changes of vital signs and laboratory examination indexes were also observed. Results ①The time, degree of vital signs recovery of the test group were superior to those of the control group, and the differences were statistically significant ( P 〈0. 05). ② The degree that NF-κB moved into the nucleolus and the TNF-a level of the control group were higher than those of the test group, and the differences were sta- tistically significant ( P 〈0. 01 ). ③Shengmai Injection suppressed the activation of NF-tcB and the blood serum level of TNF-a on dosage ( P 〈 0. 01 ). ④ The NF-κB activation degree and the TNF-a level were negatively correlated with the recovery of vital signs. The better the vital signs recovered, the lower were the NF-KB activation and the TNF-a level. Conclusion Shengmai Injection plus routine therapeutics have obvious therapeutic efficiency in protecting heart function, supporting good circulation state, and increasing the success rate of CPR possibly by suppressing the NF-κB signaling pathway and reducing NF-κB controled inflammation factors in a dose-dependent manner.
