摘  要:目的探讨儿科重症感染合并多器官功能障碍综合征(MODS)患儿血清瘦素与细胞因子水平的联动效应及其意义。方法将2002-10—2004-03就诊于武汉市儿童医院和广州市妇婴医院的31例重症感染患儿与25名健康儿童分为研究组和对照组,其中所选的重症感染患儿均合并MODS。分别采用放射免疫法测定血清瘦素、酶联免疫吸附试验检测细胞因子白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)。结果研究组的血清瘦素水平及细胞因子IL-6及TNF-α水平较正常对照组明显升高,差异具有统计学意义;在研究组,血清瘦素与IL-6及TNF-α水平变化有统计学相关性。结论儿科重症感染合并MODS患者的血清瘦素与细胞因子水平明显升高,其联动效应是宿主抗感染的重要防御反应。监测血清瘦素与细胞因子水平的变化,对儿科重症感染的进程分析及预后判断具有重要临床意义。Objective To study what happened to plasma leptin and plasma cytokines( IL-6 ,TNF-α) in the case of pediatric critical infection complicated with multiple organ dysfunction syndrome(MODS). Methods The serum samples obtained from the children with critical infection complicated with MODS and from those in the control group separately. The method of RIA was used to detect the samples for the values of leptin and ELISA for interleukin-6 ,tumor necrosis factor-α. Results The concentrations of serum leptin,IL-6 and TNF-α were significantly developed in the patients with critical infection complicated with MODS. Conclusion The levels of leptin and IL-6, TNF-α were increased significantly in the patients with critical infection complicated with MODS. The phenomenon might represent a host defense mechanism of the severe infections. The examination of the values of leptin and IL-6 ,TNF-α might be helpful in diagnosis and prognosis of the critical infection with MODS.
