摘  要:目的:探讨压力性尿失禁(SUI)患者阴道前壁组织中,核心蛋白聚糖(DCN)和转化生长因子β1 TGF-β1 mRNA表达水平与Ⅰ型胶原含量变化。方法:将40例绝经前后妇女根据是否存在SUI分为SUI组(23例)和对照组(17例)。应用酶联免疫法检测两组妇女阴道前壁组织中Ⅰ型胶原含量;实时定量PCR技术测DCN和TGF-β1 mRNA表达水平。结果:绝经前后SUI患者阴道前壁组织中Ⅰ型胶原含量减少,与对照组比较,差异有非常显著性(P<0.01);DCN mRNA表达水平增强,与对照组比较,差异有显著性(P<0.05);TGF-β1 mRNA表达水平减少,与对照组比较,绝经前组差异有显著性(P<0.05),但绝经后组,无差异(P>0.05)。结论:①阴道前壁组织中Ⅰ型胶原含量减少,可能是SUI发生的原因之一;②SUI患者阴道前壁组织中Ⅰ型胶原含量减少可能是由于局部DCN mRNA表达增强,TGF-β1 mRNA减少所致;③DCN可能通过与TGF-β1共同影响结缔组织的胶原含量参与SUI的发生。Objective: To observe the alteration of type Ⅰ collagen concentration as well as the mRNA expresssion of decorin and TGF- β1 in paraurethral connective tissue in patients with stress urinary incontinence(SUI). Methods:Transvaginal biopsies were obtained from the paraurethraJ connective tissue in 23 SUI and 17 non - SUI women. The concentration of type Ⅰ collagen was determined by ELISA. Relative real - time quantification PCR were used to determine the mRNA expressions of decodn and TGF - beta1. Results: The mean concentration of type Ⅰ collagen was significantly reduced in patients with SUI,compared to the non- SUI group(P〈0.01), while the expression of decorin was significantly higher in patients with SUI( P〈 0.05). The expression of TGF-β1 decreased in SUI patients, but the decrease was significant for premenopausal women not for postmenopausal women. Conclusions: Collagen metabolism in SUI is altered by clecorin and TGF- beta1, resulting in a less tensile strength of the paraurethral connective tissue, which might involve in the pathogenesis of SUl.
