摘  要:目的探讨肾小球滤过率(GFR)在高血压肾损害与年龄、平均动脉压(MAP)、尿微量白蛋白(MAU)、尿α1微球蛋白(α1-MG)、尿N-乙酰-β-D葡萄糖苷酶(NAG)、血肌酐(Scr)等指标间的相关性。方法高血压患者按病程分A组32例,病程5~10年;B组33例,病程>10年;C组30例,正常对照组。测定上述指标并进行组间比较及多元回归分析。结果三组间MAU、α1-MG、GFR、Scr存在显著性差异(P<0.01)。多元回归分析显示,A组GFR与MAU、α1-MG、NAG、年龄均显著相关(P<0.01),而B组GFR与MAU、Scr年龄显著相关(P<0.01)。结论联合测定α1-MG、NAG、MAU等指标可早期判定高血压肾损害。Objective To investigate the relationship among the factors of age, mean arterial pressure ( MAP ), microalbuminuria ( MAU ), α1-microglobulin ( α1-MG ), β-N-acetyglocosamidase (NAG), serum creatinine(Scr)and the glomerular filtration rate(GFR)in patients with early moderate and end stage of hypertensive nephropathy. Methods According to the duration of diseases, 65 patients with essential hypertension(EH)were classified into groups A with the duration of 5-10 years and group B with the duration greater than 10 years. MAP, GFR, MAU, α1-MG, NAG, and Scr were measured and studied by independent samples T test between the two groups. Multi-variations regression analysis was used to find the related factors in different duration of EH with GFR. Results The values of age,MAU,GFR and Scr of group A were significantly different from those of group B (P〈0. 01). Regression analysis showed that GFR was positively correlated with MAU, α1-MG, NAG in group A(P〈0. 01), but not in group B. Conclusion MAU, α1-MG, NAG could be used as early observe indexes in patients with hypertensive nephropathy.
