苦参碱葡萄糖注射液联合CHOP方案治疗非霍奇金淋巴瘤的临床研究

摘　　要：目的:比较苦参碱葡萄糖注射液联合CHOP方案和单纯CHOP方案治疗Ⅳ期非霍奇金淋巴瘤(NHL)的疗效及不良反应。方法:40例入选的Ⅳ期NHL患者分为两组,观察组22例,采用苦参碱葡萄糖注射液联合CHOP方案(环磷酰胺、强的松、长春新碱、阿霉素)化疗,平均化疗5.8周期;对照组18例,单纯用CHOP方案化疗,平均化疗5.4周期。定期检查血常规、骨髓及肝、肾功能等。结果:观察组完全缓解10例,部分缓解5例,稳定5例,进展2例,总有效率为68.18;对照组完全缓解6例,部分缓解4例,稳定5例,进展3例,总有效率为55.56,观察组的有效率提高但与对照组比较差异无统计学意义(P>0.05)。观察组化疗后出现胃肠道反应、肝功能损害及白细胞降低等较对照组减轻(P<0.05),生活质量优于对照组(P<0.05)。结论:苦参碱葡萄糖注射液联合CHOP方案能够降低化疗毒副反应,增强化疗疗效,改善生活质量,可用于Ⅳ期NHL患者的治疗。AIM： To compare the efficacy and adverse reaction of chemotherapy with CHOP protocol （CT- CHOP） alone and comhined with Matrine Glucose Injection to treat Ⅳ non-Hodgkin＇ s lymphoma （NHL）. METHODS： 40 patients were divided into 2 groups. The treatment group （n = 22） was treated with CT-CHOP plus Matrine Glucose Injection for 5.8 cycles on average and the control group （n = 18） was treated with CT-CHOP alone for 5.4 cycles on average. 2 groups were tested regularly for blood count, marrow, liver and renal function. RESULTS： In the treatment group, 10 patients had complete remission （CR）, 5 partial remission （PR）, 5 unchanged （NC）, 2 progressively deteriorated （PD）, with 68.18% total effective rate, while in the control group, 6 patients had CR, 4 PR, 5 NC, and 3 PD, with a total effective rate of55.56% （P 〉0.05）. After treatment, the quality of life （QOL） in the treatment group was improved more obviously than that in the control group （P 〈 0.05） ,and the incidence rate of adverse reaction, such as, the gastrointestinal reaction, impaired liver function, and the degrade white blood cell was h）wer （ P 〈 0.05）. CONCLUSION ： Matrine Glucose Injection therapy combined with CT-CHOP on IV non-Hodgldn＇s lymphonla can strengthen the therapeutic effect, reduce the adverse reaction of chemotherapy and improve QOL of patients.
