摘 要:目的通过中性粒细胞碱性磷酸酶(NAP)染色,探讨各种不明热病人NPA活性和阳性指数用来诊断及鉴别诊断的价值,以备早期及时诊断和治疗。方法应用快速偶氮联法对1103例,50多种不同疾病人进行NPA活性的阳性指数检测,并分组配对与正常组进行参比检测,经统计学处理,算出相关性、敏感性的特异性。结果在贫血组中,其中再障(AA),AA-PNH综合征,酶的活性明显高于其他贫血和正常组,相互之间有显著差异,P<0.01;在骨髓增殖性疾病(MPD)组中,真性红细胞增多征(PV),骨髓纤维化(MF)酶的活性较其他增殖性疾病和正常组明显增高,P<0.01;白血病组,急淋(ALL),慢淋(CLL),毛白(HCL),浆白(PCL),酶的活性较其他白血病和正常组明显增高,P<0.01,而急非淋M1、M2、M4、全髓性白血病(PMS)酶的活性最低;肿瘤组中,骨髓瘤(MM)酶的活性增高最明显,其次是非霍奇金淋巴瘤(NHL),而恶性组织细胞病(MH)酶的活性极度减低或消失为零,相互之间有显著差异P<0.01;在自身免疫性血小板减少性紫癜(AITP),全血细胞减少、粒细胞减少、血小板减少和脾亢等疾病中,除了AITP较正常不同程度增高,但经统计学处理,无明显差异,P>0.05;在感染疾病中,各种球菌感染,以及所致的粒细胞型类白细胞反应,酶的活性最高,立克次体次之,而病毒感染酶的活性较其他杆菌、寄生虫及正常组显著减低,相互之间有显著差异,P<0.01;其他疾病,如胶原病酶的活性较正常组有不同程度减低,而糖尿病不同程度增高,但经统计学处理,无明显差异,P>0.05。结论NAP对不同疾病人早期诊断及鉴别诊断敏感性和特异性诊断指标。Objective Passing the neutyophil alkaline phosphatase(NAP)dyes, inquirying into every kind of have fever male rate in NAP in patient and male index numbers examine a partient and discrminate the diagnostic value, toing have the earlier period on time examine a patient with curo.Methods Apply the fleetness accidentally nitrogen accidentally contact method to 1103 example, proceeding the NAP activity and the male index number to examine, with normal a contrast, was learned by covariance to handle,compute the relativity,sensitivity and particbdarity. Results At the anemia set,AA,AA-PNH synthesizing symptom,enzyme obvious higher than other anemia and normal set,mutually it shows the obvious difference, P 〈 0.01;in the MPD set,The activity of enzyme,the PV, MF is higher than other MPD, and normal set, P 〈 0.01 ; in leukemia set, The activity of the enzyme, the ALL, CLL, HCL, PCL is higher than other leukeamia and normal set, P 〈 0.01 ; But, the M1, M2, M4, PMS the enzyme activity is lowest; in tuner set, The activity of the enzyme,the MM increases high most obvious,the next in order is that NHL,but the MH turns down extremely or disappear for zero, mutually show the remarked difference, P 〈 0.01 ;AITP, the whole blood cell reduces, a cell reduces, the blood platelets reduces, HS etc. aparting from the normal and diffeent degree in AITP increase high, but was learned by covariance to handle, hace no obvious difference, P 〉 0.05; In infect with disease, The activity of the enzyme, every kind of ball germ infection, and the white cell leukomoid reaction, is tallest, the next is Rickettsia, But hte virus infection compares rod bacteria, parasite and normal sets turns down obviously, mutually it shows the remarked difference, P 〈 0.01 ; Other disease, have the different degree to turn down such as the normal matched contyol in activity of the immune disease in enzyme,but the different degree in diabetes increases high,but was learned by covariance to handle, have no the obvious difference, P 〉 0.05.
