摘  要:目的探讨血管内皮生长因子-D和D2-40阳性表达的淋巴管密度在膀胱移行细胞癌发生发展中的意义。方法采用免疫组织化学SP法和组织芯片技术测定15例正常膀胱粘膜组织和53例膀胱移行细胞癌组织血管内皮生长因子D的表达,用SP法标记D2-40并测定淋巴管密度。结果血管内皮生长因子-D的表达与组织学分型(P<0.01)、临床分期(P<0.01)、有无淋巴转移(P<0.01)有关;膀胱移行细胞癌组织中边缘区淋巴管密度高于中心区和正常膀胱粘膜(P<0.01);肿瘤边缘区淋巴管密度与组织学分型(P<0.01)、临床分期(P<0.01)、有无淋巴转移有关(P<0.01);另外,血管内皮生长因子-D与淋巴管密度之间存在正相关性(P<0.01,r_s=0.747)。结论血管内皮生长因子-D阳性表达及肿瘤边缘区淋巴管密度与膀胱移行细胞癌的发生发展密切相关,血管内皮生长因子-D可能促进淋巴管的新生。Objective To detect the expression of vascular endothelial growth factor-D (VEGF-D and investigate lymphatic vessel density (LVD) with positive D2-40 expression in order to confirm the roles of LVD in the growth, progression and lymph node metastases of bladder transitional cell carcinoma (BTCC). Methods The expression of VEGF-D was detected by SP immunohistochemical method and tissue microarray and the expression of D2-40 was detected by SP immunohistochemical method in 15 normal bladder tissues and 53 samples of BTCC, and LVD was counted. Results The expression of VEGF-D was associated with tumor grade (P 〈0.01), clinical stage (P 〈0.01) and lymph node metastasis (P 〈0.01). LVD within the marginal zone was higher than that in intra-tumor and normal bladder tissue (P 〈0.01) and positively correlated with tumor grade (P 〈0.01), clinical stage(P 〈0.01) and lymph node metastasis (P 〈0.01). There was a positive correlation between the expression of VEGF-D and LVD (P 〈0.01, rs=0.747). Conclusion Expression of VEGF-D and LVD within the marginal zone of BTCC are concerned with its growth, progression and lymph node metastasis, and VEGF:D may promote lymphangiogenesis process.
