摘 要:目的探讨舒氧康对急性肺损伤(ALI)家兔肺组织基质金属蛋白酶-8(MMP-8)表达的影响及其对肺损伤的治疗作用。方法将30只家兔随机分为对照组(A组),肺损伤组(B组)和舒氧康组(C组),每组10只,B和C组采用特制多功能撞击机制成急性肺损伤模型,C组撞击后经耳缘静脉给予舒氧康0.3 ml/(kg.min),分别在0、0.5、1、24、h时间点行血气分析,4 h后处死动物,取肺组织测定肺水含量、肺体质量比值及观察病理变化,免疫组化法检测MMP-8表达。结果与A组相比,B组损伤后PaO2、SpO2显著降低(P<0.05),肺组织MMP-8表达显著升高(P<0.05),肺水含量及肺体质量比值显著增高(P<0.05),镜下见肺间质、肺泡水肿,大量炎性细胞浸润。C组经舒氧康治疗后,与B组比较,PaO2、SpO2明显升高(P<0.05),肺组织MMP-8表达降低(P<0.05),肺水含量及肺体质量比值无明显增加,肺水肿减轻。结论舒氧康对外伤性肺损伤有治疗作用,其机制可能是通过降低肺组织MMP-8表达实现的。Objective To study the effects of oxygenator on the expression of matrix metalloproteinases (MMP-8) in lung tissue and its potential functions in rabbits with acute lung injury (ALI). Methods Thirty rabbits were randomly divided into three groups: control group (group A), ALI group (group B) and oxygenator treatment group (group C) with 10 in each group. ALI model was established by special equipment in group B and group C, and oxgenator was applied in group C [0.3 ml/(kg · min)]. Blood samples were taken [or blood-gas analysis at 0, 0.5, 1, 2 and 4 h alter trauma. Four h later, the animals were killed, and the expression of MMP-8 in lung tissue was detected by immunohistochemistry, the content of lung water and lung/body ratio were measured, and pathologic changes of lung tissue were observed microscopically. Results Compared with group A, PaO2 and SpO2 were significantly decreased (P〈0.05), the expression o[ MMP-8 in lung tissue, content o[ lung water and lung/body ratio significantly increased (P〈0.05), and local hemorrhage, interstitial edema, alveoli exudation, and massive inflammatory cells infiltration were also observed in group B. In group C, as compared with those in group B, PaO2 and SpO2 were signi[icantly increased (P〈0.05), the expression of MMP-8 in lung tissue significantly decreased (P〈0.05), content of lung water decreased (P〈0.05), and only mild pathological changes were observed. Conclusion Oxygenator has a certain therapeutic effectiveness on ALI by down-regulating the expression of MMP-8 in lung tissue.
