肾病患者尿Tamm-Horsfall蛋白含量变化及其临床意义

摘　　要：采用抗Ｔａｍｍ－Ｈｏｒｓｆａｌ蛋白（ＴＨＰ）的单克隆抗体ＥＬＩＳＡ法检测了１７４例肾病患者和１５２例正常人的尿ＴＨＰ含量，结果显示：肾病组尿ＴＨＰ含量明显低于正常对照组，差异显著（Ｐ＜０．０１）；９种原发性、继发性肾病患者的尿ＴＨＰ含量均分别明显低于正常对照组，差异显著（Ｐ＜０．０１）。血尿素氮（ＢＵＮ）、血肌酐（ＳＣｒ）异常的患者，其尿ＴＨＰ含量与ＢＵＮ、ＳＣｒ呈显著负相关（Ｐ＜０．０５，Ｐ＜０．０１）；而ＢＵＮ、ＳＣｒ正常的患者，其尿ＴＨＰ含量与ＢＵＮ、ＳＣｒ无明显相关性（Ｐ＞０．０５）。提示：尿ＴＨＰ含量的减少与肾脏病变密切相关，检测尿ＴＨＰ可作为判断肾脏病变及肾功能损害程度的良好指标。Measured urinary Tamm Horsfall protein(THP)contents of 174 renal patients and 152 normal persons by ELISA with anti THP monoclonal antibody.The results showed that urinary THP contents in the renal patients was lower significantly than that in the normal persons(P<0 01).Urinary THP contents in 9 kinds of primary and secondary renal patients were also lowered than that in the normal persons respectively.Urinary THP contents in the patients who′s blood urea nitrogen(BUN)and serum creatinine(SCr)were abnormal correlated negatively with BUN and SCr(P<0 05,P<0 01).But when BUN,SCr were normal in renal patients ,the urinary THP contents were not so (P>0 05).This study suggests that there is closely related between the decrease of the urinary THP contents and renal disease.Urinary THP may be used as a new sensitive and reliable index of renal function.
