注射用血凝酶对耻骨上前列腺摘除术患者凝血功能的影响

摘　　要：目的探讨注射用血凝酶在耻骨上前列腺摘除术的止血效果。方法2003年3月-2006年3月140例（血凝酶组）良性前列腺增生患者行耻骨上前列腺摘除术,术中、术后应用注射用血凝酶止血,并与同期未使用止血药物的160例患者（对照组）比较。结果血凝酶组术中、术后出血量均低于对照组（P〈0.05）。血凝酶组输血2例,平均（200.0±20.0）ml;对照组10例,平均（387.5±105.2）ml,组间比较有显著差异（P〈0.05）。血凝酶组膀胱冲洗时间平均（1.5±1.4）d,明显短于对照组的（4.5±1.5）d（P〈0.05）。术后24 h血凝酶组凝血酶原时间和活化部分凝血酶时间明显少于对照组（P〈0.05）。结论耻骨上前列腺摘除术中、术后应用血凝酶,可有效止血,减少输血量。Objective To investigate the hemostatie effect of hemocoagulase in suprapubie prostateetomy. Methods From March 2003 to March 2006, 140 cases of benign prostatic hyperplasia （hemocoagulase group） underwent suprapubic prostatectomy were administted hemocoagulase for injection to hemostasis. 160 synchronized benign prostatic hyperplasia cases underwent the same operation were compared as control group. Results The intra- and post- operative blood loss of hemocoagulase group was less than that of control group （P 〈0.05）. Two cases of hemocoagulase group accept transfusion with average volmne of （200.0 ±20.0） ml, and 10 cases of control group with （387.5 ±105.2） ml. There was significant difference between two groups （P〈0.05）. The average duration of bladder irrigation of hemocoagulase group was （ 1.5 ±1.4）d, and obviously shorter than （4.5 ± 1.5）d of control group （P 〈0.05）. The prothrombin time and activated partial thromboplastin time of hemocoagulase group within 24 hour after operation was remarkably shorter than that of control group（P 〈 0.05）. Conclusions Administration of hemocoagulase can control bleeding efficienfly and reduce blood transfusion significantly in suprapubic prostatectomy.
