PPH与外剥内扎术治疗混合痔并重度内痔对比观察

摘　　要：探讨吻合器痔上黏膜环切术（PPH）结合外剥内扎术与单纯外剥内扎术治疗混合痔并Ⅲ，Ⅳ度内痔的疗效。PPH组44例行PPH后，再对需要切除的痔核行外剥内扎术，剥扎组47例单纯行外剥内扎术。PPH组在减轻术后疼痛及创缘水肿、缩短愈合时间、减少术中术后出血及避免肛门狭窄方面优于剥扎术组。结果表明，PPH结合外剥内扎术治疗混合痔并Ⅲ，Ⅳ度内痔优于外剥内扎术，为PPH在临床上的应用拓宽了思路。The therapeutical results of PPH plus external dissection ＆ internal ligation （EDIL） and only external dissection ＆ internal ligation in the treatment of mixed hemorrhoids and severe internal hemorrhoids were observed and analized. Forty four patients underwent PPH and neccessary EDIL （the trial group） and 47 patients underwent EDIL alone. As results,the combined-procedure of PPH plus EDIL was better in such aspects as postoperative pain, edema, healing time, blood loss and anal stricture. It is concluded that PPH plus EDIL is better than only EDIL in the treatment of mixed hemorrhoids and internal hemorrhoids of Ⅲ , Ⅳ stages.
