摘  要:目的观察足跟采血疼痛刺激时新生儿生理指标、疼痛评分的变化及非药物止痛的效果。方法100例足跟采血的足月新生儿随机分为五组,各20例。对照组:穿刺前两分钟不作任何处理;安慰剂组:穿刺前两分钟喂以消毒蒸馏水;葡萄糖(glucose,GS)组:穿刺前两分钟喂以葡萄糖溶液;非营养性吸吮(non-nutritive sucking,NNS)组:在穿刺前两分钟将安慰奶嘴放到口内;GS+NNS组:喂以葡萄糖溶液后给安慰奶嘴。监测各组哭持续时间及穿刺前后心率、呼吸频率、平均血压、经皮血氧饱和度的变化。采用新生儿急性疼痛评分表(Douleur Aigue Nouveau-ne scale,DAN)及早产儿疼痛评分简表(preterm infant pain profile,PIPP)进行疼痛评价。结果(1)穿刺后15S,对照[(157.6±14.O)次/min]、安慰剂[(159.2±11.1)次/min]、GS[(146.6±13.O)次/min]、NNS[-(148.4±13.3)次/min]、GS+NNS组心率[(147.9±11.8)次/min]均快于穿刺前,分别为(132.1±17.8),(132.8±15.4),(134.2±13.3),(132.0±15.6)和(134.1±17.5)次/min(P〈0.01);穿刺后1min,对照组和安慰剂组心率仍快于穿刺前(P〈O.05),而GS、NNS和GS+NNS组心率已恢复到穿刺前水平(P〉0.05)。(2)穿刺后1min,对照、安慰剂、GS、NNS、GS+NNS组呼吸频率均快于穿刺前(P〈O.05);穿刺后2和3min,对照组及安慰剂组呼吸频率仍快于穿刺前(P〈O.05),而GS、NNS、GS+NNS组已恢复到穿刺前水平(P〉0.05)。(3)穿刺后GS和GS+NNS组哭持续时间为(1.65±3.12)S和(1.65士5.26)S,均短于对照、安慰剂和NNS组的(21.60±27.49)S、(17.60士17.45)S和(10.20±10.92)S(P〈O.01)。(4)GS、NNS和GS+NNS组的DAN和PIPP评分均低于对照组及安慰剂组(P〈O.01)。结论GS、NNS和GS+NNS均有止痛作用�Objective To observe the physiological changes, behavior score and the effects of non-pharmacologic analgesia in newborns during heel blood collecting. Methods 100 term newborns were randomly divided into five groups, including control group, placebo group, glucose (GS) group, non-nutritive sucking (NNS) group and glucose plus non-nutritive sucking (GS+NNS) group. Heart rate, respiratory rate, oxygen saturation, mean blood pressure before and after heel prick were compared and duration of crying after heel prick were compared among five groups. The pain was assessed with Douleur Aigue Nouveau-ne scale (DAN) and preterm infant pain profile (PIPP). Results (1) Heart rate of neonates in control [(157. 6±14.0) bpm)],placebo [(159.2±11.1) bpm],GS[(146.6=t=13.0) bpm],NNS [(148.4!13.3) bpm] and GS+NNS group[(147.9 11. 8) bpm]at 15 seconds after heel stick were significantly higher than those before heel stick [(132.1±17.8), (132. 8±15. 4), (134. 2±13. 3), (132. 0±15. 6) and (134.1±17. 5) bpm] respectively(P〈0.01). One minute after heel prick, the heart rate in the three intervention groups (GS, NNS, GS+NNS) returned to normal level, while that in the control group and placebo group was still higher than normal level(P〈0.05). (2) Respiratory rate of neonates in control, placebo, GS,NNS,GS+NNS group at 1 minute after heel stick was significantly higher than that of before heel stick (P〈0. 05). Two and three minutes after heel prick, respiratory rates in intervention groups returned to normal level, while that in the control group and placebo group was still higher than normal level(P〈0.05) . (3)The durations of crying in GS[(1.65±3. 12) s] and GS+NNS group [(1.65±5.26)s] was significantly shorter than that of control group [(21.60±27.49)s], placebo group [(17.60±17.45)s] and NNS group [(10.20±10.92)s](P〈0.01).
