摘 要:目的探讨Sysmex-2100光学法血小板计数作为低血小板标本复栓方法的可行性。方法对60例Sysmex-2100血细胞全自动分析仪阻抗法检测PLT值在(0~1100)×10^9/L的标本进行流式仪计数法、光学法、阻抗法、手工法的检测,以流式仪计数法作为参考方法进行比较。对100例阻抗法检测PLT〈60×10^9/L的标本进行光学法、阻抗法、手工法的检测并比较。结果对60例标本PLT检测的流式仪计数法和光学法、阻抗法、手工法3种方法的结果进行回归分析,3种方法跟流式仪计数法的相关系数分别为0.992,0.989和0.985,均高度相关(P〈0.001)。分组(PLT〈100、100~300、〉300×10^9/L)进行两样本配对t检验,在低PLT组阻抗法和流式仪计数法有统计学差异(P〈0.05),在高PLT组阻抗法和手工法同流式仪计数法均有统计学差异(P〈0.05)。对100例PLT〈60×10^9/L标本的光学法、阻抗法、手工法结果进行回归分析,阻抗法和手工法对光学法结果相关系数分别为0.752和0.882。t检验结果是阻抗法同光学法有统计学差异(P〈0.01),而手工法同光学法没有统计学差异(P〉0.05)。结论阻抗法同流式仪计数法检测结果在血小板值低的时候有统计学差异,该误差可能影响临床诊治。而光学法在很大PLT值范围内同流式仪计数法相关性很好,且比阻抗法和手工法准确。因此光学法的准确性和重复性都较好,并且成为简便快捷的血小板复查方法,在标本量较大的实验室值得采用。Objective To study feasibility of optical platelet counts with Sysmex XE-2100 as a recheck method in low platelet samples. Methods 60 blood samples with platelet counts form 0 to 1 100X 109/L in resistance method were counted with resistance method, optical method, manual method and the RBC/platelet ratio reference method, and analyzed the correlation between the results of the 3 former methods and the latter one statistically. 100 blood samples with platelet counts less than 60 × 10^9/L in resistance method were counted with optical method (comparison method), resistance method and manual method and results were compared. Results The correlative coefficients of the 3 methods of 60 samples were 0. 992,0. 989 and 0. 985 ,respectively(P〈 0. 001 ). Paired sample t-test indicated significant difference in platelet counts less than 100 × 10^9/L between resistance method and reference method,and in platelet counts more than 300 × 10^9/ L in resistance method between reference method,and manual method between reference method,too(P〈0. 05). The correlative coefficients of the 2 methods of 100 samples were 0. 752 and 0. 882,respectively. Paired sample t-test showed significant difference in platelet counts less than 100× 10^9/L between resistance method and optical method(P〈0. 01),and no significant difference between manual method and optical method(P〉0. 05). Conclusion Platelet counts of resistance method showed significant difference with those of RBC/platelet ratio reference method in low platelet samples. And the optical method showed good accuracy and high correlative coefficient with reference method in a large PLT range. Optical method is a recommendable PLT recheck method in laboratory with large sample magnitude.
