晚期脊髓损伤病理改变的临床研究

摘　　要：目的对晚期脊髓损伤患者的病理进行临床研究,探讨雪旺细胞移植治疗晚期脊髓损伤的手术方法及疗效。方法术中显微镜下观察脊髓形态学改变及囊腔内容物组织学检查。其中男38例,女9例,年龄为2～58岁,伤后时间为3～216个月。结果47例患者均存在不同程度的硬脊膜瘢痕增生、脊髓表面蛛网膜粘连、增厚、疤痕组织、脊髓血管减少、苍白、萎缩、无波动,其中90%患者合并脊髓囊腔或空洞。囊腔内容物组织学检查均为液化、坏死组织。结论对晚期脊髓损伤的病理研究,能帮助晚期脊髓损伤的雪旺细胞移植的手术方法改进及提高疗效。The present clinical study of pathological changes of the chronic spinal cord injury was undertaken to assess the operation method and curative efficacy of fetal human Schwann cell （SC） transplantation into the paraplegic patients＇spinal cord suffered from chronic trauma. Methods The morphological changes of injured spinal cord were observed under an operation microscope. Moreover, the cavities or its cysts were examined histologically. These cases consist of 38 male and 9 female paraplegic patients aged from 2 to 58 years with an insulted duration of 3 to 216 months after SCI. Results The normal pulse of spinal cord of 47 cases could not be observed, and the thickened arachnoid membranes exterior to spinal cord oecarred, damaged dura tissues sticked to each other, and glial sear tissues formed at different extent. Furthermore, the number of blood vessels of spinal cord was decreased, spinal cord tissue became pale, myelomalacial, and atrophied in the area of injury in all cases. As well pathological cysts contained a thick translucent wall and turbid liquid could also be detected in the area of injury in 90% of cases, and these eases presented myelosyringosis. Conclusion The clinical study on pathological change of the chronic spinal cord injury can be helpful to improve the surgical methods by transplanting fetal human SC into SCI paraplegic patients.
