摘  要:目的:应用组织多普勒Tei指数(TDI-Tei)评价冠状动脉(冠脉)不同狭窄程度对左室总体心功能的影响。方法:无心肌梗死病史并接受冠脉造影(CAG)患者70例,根据CAG分为3组:A组30例,冠脉狭窄<50%;B组20例,冠脉狭窄在50%~69%之间;C组20例,冠脉狭窄≥70%。分别应用组织多普勒(TDI)和脉冲多普勒(PW)检测Tei指数。比较二尖瓣环3个不同部位TDI-Tei之间差异及与脉冲多普勒Tei指数(PW-Tei)的相关性。结果:平均TDI-Tei在A组、B组和C组依次为0.44±0.06,0.46±0.06,0.53±0.07,其中C组显著高于A组和B组(均P<0.05),B组与A组间差异无统计学意义(P>0.05)。3个不同取样部位TDI-Tei均与PW-Tei显著相关(均P<0.05)。结论:TDI-Tei可检测出冠脉高度狭窄引起的左心总体功能障碍,具有较高敏感性及特异性。Objective: To determine the value of tissue Doppler imaging (TDI) -Tei in evaluating left ventricular cardiac function in patients with different grades of coronary artery stenosis. Methods: Seventy patients receiving coronary angiography (CAG) were divided into 3 groups, patients with coronary artery stenosis 〈 50%(group A, n=30), patients with coronary artery stenosis 50%-69%(group B,n=20) and patients with coronary artery stenosis ≥70%(group C,n=20). All patients received uhrasonic cardiograph (UCG) and their TDI-Tei and pulsed-wave Doppler tissue imaging (PW-Tei) indexes were examined. The relationship between TDI-Tei and PW-Tei and the differences of TDI-Tei among 3 different sample locations were evaluated. Results: The values of TDI-Toi were significantly higher in group C. There was no significant difference between group A and B. There were significant correlations between TDI-Tei and PW-Tei in 3 different sample locations (P 〈 0.05,respectively). Conclusion: TDI-Tei can reflect the impairment of global left ventricular cardiac function in patients with severe coronary artery stenosis.
