苦参素联合中药治疗慢性丙型病毒性肝炎临床观察

摘　　要：目的:观察苦参素联合中药辨证治疗慢性丙型肝炎肝郁脾虚证患者的抗病毒疗效。方法:中医辨证为肝郁脾虚证的慢性丙型肝炎患者48例,治疗组28例,苦参素联合中药辨证治疗;对照组20例,IFNα-2b联合利巴韦林;治疗12、24、48周和治疗后24周随访时,观察病毒应答情况、肝功能。结果:43例患者完成全部治疗(治疗组25例,对照组18例)。治疗组SVR为44%(11/25),对照组为38.89%(7/18),两组比较差异无显著性意义(P>0.05);治疗组72周ALT复常率高于对照组,两组比较差异有显著性意义(P<0.05)。5例患者脱落,治疗中无严重不良反应。结论:苦参素联合中药辨证治疗能有效抑制HCV RNA复制,可改善患者肝功能,无明显严重毒副反应。Objective： To study the clinical effect of kurorinone injection in combination with syndrome differentiation of TCM to treat patients with chronic hepatitis C. Methods： Forty-eight patients, who were HCV RNA positive and anti-HCV-posi- tive and differentiated as TCM syndrome pattern of dominance of liver Qi and asthenia of spleen, were enrolled. They were divided into treated group （28 patients, receiving kurorinone injection, 400rag in qdx24w, po, bidx48w, plus Chinese medical herbs, 150ml pobidx48w） and controlgroup （20 patients, receiving interferon-alpha （2b）, 3MU, ih, qodx48w, plus ribavirin 500mg） . At the end of treatment, the virological response and biochemical response were evaluated. Results： Forty-three cases accomplished the 48 week study （25 cases in treated group, 18 cases in control group） . At the end of treatment, SVR was 44% （11/25） in treated group, and 38. 89% （7/18） in control group. There were no significant differences between them （P 〉 0.05）. The biochemical response of ALT, in 48 and 72 weeks, were 84% and 88% in treated group, and 77.78% and 61.11% in control group respectively. There was significant difference between two groups in 72 weeks （P 〈 0. 0S）. Conclusion： It seems that kurorinone injection in combination with syndrome differentiation of TCM could suppress HCV RNA replication, and help to improve liver function.
