慢性再生障碍性贫血辨证分型与TNF-α、IFN-γ水平相关性研究

摘　　要：目的探讨慢性再生障碍性贫血(CAA)中医辨证分型与TNF-α、IFN-γ水平的相关性,为CAA的中医辨证分型提供客观化依据。方法选择90例患者(脾肾阳虚型、肾阴阳两虚型、肝肾阴虚型各30例),以15例健康志愿者为对照,采用ELISA法检测其血清、骨髓单个核细胞培养上清TNF-α、IFN-γ分泌水平;RT-PCR法检测骨髓单个核细胞TNF-α、IFN-γmRNA表达水平。结果患者血清、骨髓单个核细胞培养上清TNF-α、IFN-γ分泌水平均明显高于正常组(P<0.05,P<0.01);患者三组按照脾肾阳虚组、肾阴阳两虚组、肝肾阴虚组顺序,其TNF-α、IFN-γ水平逐渐升高,两两比较差异显著(P<0.05);TNF-α、IFN-γmRNA表达水平,脾肾阳虚组最低,肝肾阴虚组最高,而肾阴阳两虚组介于两者之间。结论TNF-α、IFN-γ可以作为CAA中医辨证分型的客观化依据。Objective To investigate the correlativity between Chinese medical syndromes of chronic aplastic anemia（CAA） and the levels of TNF-α and IFN-γ of CAA, in an attempt to objectify the differentiation syndrome of CAA. Methods Ninety cases were differentiated into three groups and 15 healthy people were enrolled as control. The levels of TNF-c~ and IFN-γ of cultural supernatant of CAA patients＇ serum and marrow mononuclear cell was detected by ELISA technique; and the expression level of TNF-α and IFN-γ mRAN of CAA patients＇ marrow mononuclear cell was detected by RT-PCR technique. Results The secretion level and expression level of TNF-α and IFN-γ were obviously higher than the normal group; and they gradually rose according to the sequence of spleen-kidney yang deficiency, kidney yin- yang deficiency, and liver-kidney yin deficiency, with differences between any two syndromes （P〈 0.05）. Conclusion TNF-α and IFN-γ can be used as the objective indexes for syndrome differentiation of CAA.
