摘　　要：目的检测急性冠脉综合征(ACS)患者CD4+T细胞CD40L的表达率、血清可溶性CD40配体(sCD40L)的水平,探讨CD40/CD40L在ACS发病中的作用及可能途径。方法ACS患者42例,包括稳定型心绞痛(SAP)12例、不稳定型心绞痛(UAP)14例、急性心肌梗死(AMI)16例;健康对照者13例。流式细胞分析术检测CD4+T细胞表达CD40L的阳性细胞率,ELISA法检测血清sCD40L水平。结果AMI、UAP、SAP、对照组CD4+T细胞中表达CD40L的阳性细胞率分别为(8.67±3.31)%、(3.24±1.13)%、(2.29±1.93)%、(0.57±0.19)%,AMI组显著高于其他三组(P均<0.05),UAP组亦显著高于对照组(P<0.05);四组血清sCD40L水平分别为(14.53±8.08)ng/mL、(8.06±6.96)ng/mL、(7.37±3.59)ng/mL、(4.45±1.48)ng/mL,AMI组显著高于其他三组(P均<0.05)。结论ACS患者CD4+T细胞CD40L的表达率和血清sCD40L水平增高,在ACS发病过程中起重要作用;CD40L和sCD40L有望作为冠心病危险性的预测因子。Objective To measure the expression of CD40 ligand on CD4^＋ T cell and serum concentration of sCD40 ligand in the patients with acute coronary syndrome （ACS）. To investigate the effects and mechanisms of CD40/CD40 ligand in ACS. Method 42 patients with coronary heart disease were divided into stable angina pectoris group （ SAP, 12 cases ） , acute myocardial infarction （ AMI, 16 cases ） and unstable angina pectoris （ UAP, 14 cases ）. 13 healthy volunteers were control group. Expression of CD40 ligand was determined by flow cytometry （ FCM ）. Serum concentration of sCD40 ligand was determined by ELISA. Result The expression percentage （ % ） of CIM0 ligand on CD4^＋ T cell in AMI, UAP, SAP, and control group were （8.67 23.31） %, （3.24 21.13） % ,（2.29 21.93）% and （0.57 ±0.19 ）%. It was significant higher in AMI than that in other groups （all P 〈 0.05 ） , and it was significant higher in UAP than that in control group （ P 〈 0.05 ）. Serum concentration of sCD40 ligand in four groups were （14.53 28.08）ng/mL,（8.06 26.96） ng/mL, （7.37 ±3.59）ng/mL, and （4.45 2 1.48 ） ng/mL. It was significant higher in AMI than that in other groups （ all P 〈 0.05 ）. Conclusion Increased expression of CD40 ligand on CD4^＋ T cells and higher serum concentration of sCD40 ligand played an important role in ACS patients. CD40 ligand or sCD40 ligand may become an independent risk indictor of coronary heart disease.
