摘 要:目的:探讨星状神经节阻滞(stellate ganglion block,SGB)在严重烧伤小鼠腹腔巨噬细胞糖皮质激素受体(glucocorticoid receptor,GR)表达及炎性细胞因子分泌中的作用,明确SGB可否成为改善严重创伤后的GR功能、抑制炎性反应过度的手段。方法:20%TBSAⅢ度烧伤小鼠分为单纯严重烧伤组(烧伤组)、严重烧伤后单侧颈部SGB治疗组(单侧SGB组)、严重烧伤后双侧颈部SGB治疗组(双侧SGB组),另设正常对照组,比较各组巨噬细胞GR水平及巨噬细胞分泌炎性细胞因子肿瘤坏死因子-α(TNF-α)、白细胞介素-1"(IL-1β)、白细胞介素-6(IL-6)的差异。结果:单侧及双侧SGB组腹腔巨噬细胞GR蛋白水平均显著高于烧伤组,单侧及双侧SGB组腹腔巨噬细胞分泌TNF-α、IL-1β、IL-6的作用均显著低于烧伤组。严重烧伤后单侧SGB组及双侧SGB组间巨噬细胞GR蛋白水平及TNF-α、IL-1β、IL-6的表达释放无明显差异。结论:单侧或双侧颈部SGB均可成为改善严重烧伤巨噬细胞GR功能、抑制其过度分泌炎性细胞因子的手段。Objective To explore the roles of stellate ganglion block (SGB) in glucocorticoid receptor (GR) expression in the peritoneal macrophage and the release of proinflammatory cytokine releases by the cell in mice with severe burn injury and judge whether SGB can be an approach for improving the GR function and suppressing the excessive inflammatory response. Methods The mice with 20% TBSA HI burn injury were randomly assigned to receive no treatment, unilateral SGB, or bilateral SGB. The other heahhy mice were enrolled as normal control. GR protein levels in the peritoneal macrophage and the differences in the release of proinflammatory cytokines (TNF-α, IL-1β, and IL-6) by the cell in each group were compared. Results As compared with the mice without treatment, the GR protein levels in the mice receiving unilateral or bilateral SGB were significantly higher but the release of TNF-α, IL-1β, and IL-6 was markedly lower. There were no significant differences in the GR protein levels and the release of TNF-α, IL-1β, and IL-6 between the mice with unilateral SGB and those with bilateral SGB. Conclusion SGB can be a therapeutic approach for ameliorating GR function in macrophage and inhibiting the excessive release of proinflammatory cytokines by the cell following severe burn injury.
