内皮素-1在腰椎间盘突出中的表达及意义

摘　　要：目的检测内皮素-1在突出椎间盘与相应神经根组织中的表达情况，探讨其致腰腿痛发病的机制。方法手术制作猪腰椎间盘突出的模型，设立正常对照组（A组）、手术对照组（B组）和椎间盘突出组（C组），采用免疫组织化（ABC）法检测突出椎间盘和相应神经根组织在不同时间点（3、14、28d）内皮素-1表达，并用Mias-2000自动图像分析系统量化测量结果，对A、B、C组比较。结果A、B组椎间盘及相应神经根组织中ET—1有弱而少的表达，C组3d、14d、28d组有强的表达，C组与A、B组比较均有统计学意义（P均〈0．05）；C组中3d组表达与本组14d、28d组比较灰度值低，但比较无统计学意义（P〉0．05）。结论内皮素-1参与腰椎间盘突出后的病理生理过程，可能是腰椎间盘突出后重要的致痛及致损伤因子。Objectives To investigate the expression of endothelin - 1 （ ET - 1 ） in the herniated lumbar disc and the corresponding nerve roots, and to explore the mechanism of the herniated disc of the lumbar spine leading to seiatlea. Methods Twenty one swines were employed and were divided into three groups： group A with 3 swines （normal control group）, group B with 9 swines （narmal disc group ）, and group C with 9swines （herniated lumbar disc group）. In group A, the discs and nerve roots were harvested directly for measurement, and in group B, the operations were carried out to expose the interveterheral discs hut the annuli fihrosus were kept intact. While in group C, the neuelei pulposus were exposed to make sure the discs were protruding In the latter two groups, the discs and corresponding nerve roots were taken out 3,14, 28 days after operation. Immunohistoehemistry were employed to observe the expression of ET - 1 in the herniated discs of the nuclei pulposus and the corresponding nerve roots. The stained sections were measured in intensity by Mias -2000 image analysis system and were compared. Results ET - 1 was expressed weakly and partly in the herniated lumbar discs and the corresponding nerve roots of swines in group A and B, and were intensively overexpressed in group C. There were statistically significant difference between group C and both group A and group B （P 〈0.05 ）. In group C ,discs of 3 days after operation showed low gray scale values, those of 14, 28 days after operation showed high gray scale value, while there were no significant difference between each of the three groups. Conclusion ET - 1 plays a critical role in the pathophysio- logical development of the herniated disc of lumbar spine and is one of factor eorelated with baekleg pain and in the herniated discs of lumbar.
