摘　　要：目的探讨T3期声门型喉癌的功能保全性手术治疗的可行性，技术操作和临床疗效。方法回顾性分析1996～2005年收治的T3期声门型喉癌45例，均行功能保全性治疗，喉垂直部分切除术40例，喉水平垂直部分切除术5例。以胸骨舌骨肌肌筋膜瓣5例，颈阔肌肌皮瓣40例。全部患者均行术后放射治疗（50～60Gy）。结果3a生存率81．8％，5a生存率68．2％，45例患者中已拔管42例，拔管率93．3％。全部患者均恢复经口进食，无呛咳、误咽，所有患者均发声成功。结论T3期声门型喉癌虽为晚期恶性肿瘤，但只要严格掌握适应证，熟练运用多种修复方法，提高外科手术技巧，保留喉功能是可行的。Objective The function which inquiries into one T3 period glottic carcinoma of the larynx conservation surgery the possibility of sex treatment, technique operation and clinical curative effect. Methods Look back sex analyze 1996 - 2005 years accept to cure of the T3 period glottic carcinoma of the larynx conservation be 45 cases, all go conservation surgery ,vertical partial ]aryngectomy are 40 cases, and larynx level vertical partial laryngectomy are 5 cases. Stemohyoideus flap are 5 cases and myocutaneous are 40 cases, All sufferers given postoperative irradiation （50 -60 C,y）. Results 3 years existence rate：81.8% , 5 years existence rate ：68.2% , have already pulled out to take care of 42 in 45 sufferers, pull out a tube rate 93.3%. All sufferers recover is take a meal by and have no Qiang to cough and the mistake swallow and all sufferers all give out sound .success. Conclusions T3 period voice door type larynx cancer although later period malignant tumor, as long as strictly control adaption, masterly make use of various repair methods, raise surgical operation technique, reserve the larynx function is viable.
