行冠脉造影患者中肾动脉狭窄的检出率及相关危险因素分析

摘　　要：目的:探讨肾动脉狭窄的检出率及相关危险因素。方法:回顾性分析2003年1月至2005年1月期间我院1049例行冠状动脉造影并同时行肾动脉造影患者的临床资料及影像学资料。结果:(1)1049例患者中肾动脉显著性狭窄(直径狭窄≥50%)的检出率13.3%。564例冠心病患者肾动脉显著性狭窄的检出率20.7%,明显高于非冠心病患者。(2)单因素分析显示:肾动脉狭窄组与无肾动脉狭窄组比较,冠心病、冠状动脉钙化、陈旧心肌梗死、三支病变及高血压患者比例明显增高,低密度脂蛋白胆固醇水平、年龄也明显偏高。(3)高龄、高血清肌酐浓度水平、冠心病特别是三支冠脉病变患者有高的肾动脉狭窄检出率。(4)多因素Logistic回归显示:年龄、血清肌酐浓度、冠心病、三支冠脉病变是预测肾动脉狭窄的独立危险因子。结论:冠心病患者中肾动脉狭窄较常见,患者的临床特征能提示肾动脉狭窄。Objective： To investigate the detection rate and predictors of renal artery stenosis （RAS） in patients undergoing renoarteriography. Methods： Retrospective analysis was made based on the clinical and angiographic data of patients undergoing renal arteriography after performing coronary arteriography during January 2003 and January 2005. Results： （ 1 ） Detection rate of significant RAS was 13.3% in 1049 patients. The rate was 20.7% in 564 patients with coronary heart disease（CHD）, which was significantly higher than that in patients without CHD. （2） Single factor analysis showed that the proportion of CHD, coronary artery calcification, old myocardial infarction, three - vessel coronary artery stenosis and hypertension , the level of low density lipeprotein cholesterol （LDL - C） and age were higher in RAS patients than those in controls. （3） Higher detection rates of significant RAS were found in patients with elder, high level of LDL - C, CHD and three - vessel coronary artery stenosis. （4）Muhi - variance logistic analysis showed that age, serum creatinine, CHD and three - vessel coronary artery stenosis were the independent predictive factors. Conclusion： RAS was a frequent finding in patients with CHD. The clinical features of patients can predict the presence of RAS.
