睡眠呼吸暂停综合征与冠心病危险因素的相关性

摘　　要：目的:探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与冠心病(CHD)的相关性。方法:选取OSAHS患者39名、OSAHS合并CHD患者37名、健康对照者40名。均进行多导睡眠监测,清晨空腹卧位静脉取血,检测。结果:OSAHS及OSAHS合并CHD患者的凝血酶原时间、血糖、甘油三酯、总胆固醇、总胆固醇/高密度脂蛋白、载脂蛋白B较对照组明显升高(P<0.05),而载脂蛋白A、高密度脂蛋白、明显低于对照组(P<0.05),组间比较亦存在差异;与睡眠监测指标进行相关分析发现血红蛋白、红细胞压积、血糖、甘油三酯、总胆固醇、总胆固醇/高密度脂蛋白与睡眠呼吸暂停/低通气指数(AHI)呈正相关(P<0.05),活化部分凝血活酶时间与AHI呈负相关(P<0.05);红细胞压积、血红蛋白与最低血氧饱和度(LsaO_2)呈负相关(P<0.05)。结论:OSAHS可引起血液有形成分、凝血功能、血糖血脂代谢等异常,导致动脉粥样硬化的发生及发展。其与CHD有明确的相关性,是独立于年龄、体重、饮食、遗传等原因的CHD的发病因素之一。Objective： To study the association of obstrctive sleep apnea - hypopnea syndrome （ OSAHS ） with coronary atherosclerotic heart disease. Methods： 40 healthy, 39 patients with OSAHS and 37 patients with OSAHS and coronary atherosclerotic heart disease received polysommography and blood tests of coagulated activity, blood glucose and blood lipid level. Results： The levels of prothrombin（PT）, glucose（GLU）, triglycefide（TG）, total cholestein（TC）, TC/HDL, apoprotein B（ApoB） in OSAHS group and OSAHS with coronary atherosclerotic heart disease group were lower than those in control group（ P 〈 0.05） ; the levels of high - density lipoprotein （HDL） and apoprotein A （ApoA） in OSAHS group and OSAHS with coronary atherosclerotic heart disease group were higher than those in control group（ P 〈 0.05）. Hemocrit （HCT） ,hemoglubin （Hgb）, GLU,TG, TC and TC/HDL correlated with AHI（ P 〈 0.05 ）. Activated partial thromboplastin time（APTT） correlated negatively with AHI（P 〈 0.05） . HCT and Hgb correlated negatively with LsaO2 （ P 〈 0.05）. Conclusion： OSAHS can lead and aggravate arteriosclerosis. It was independently associated with coronary atherosclerotic heart disease. Thus particular vigilance for OSAHS was very important.
