心房颤动华法林抗凝治疗剂量调整与监测的探讨

摘　　要：目的:探讨我国心房颤动(AF)患者华法林抗凝治疗的剂量及通过监测INR调整药物剂量的方法,推动华法林抗凝工作的开展。方法:AF患者接受华法林抗凝治疗66例,男38例,女28例,观察华法林治疗剂量,监测INR及出血等副作用。结果:华法林INR达标时间个体差异较大,华法林维持剂量范围为1.5～5mg,80.2%患者华法林用量为2～3.5mg,维持INR2～3时,轻度出血率7.6%,经相关处理并调低INR值,所有出血均停止,无1例因严重出血而终止抗凝治疗。结论:AF患者应用华法林抗凝意义重大,个体应用差异较大,但在严密监测下应用,安全可靠。Objective： To evaluate the dose of warfarin in the treatment of anticoagulation in patients with atrial fibrillation in China and adjust the dose of warfarin by examinate INR to puse the work of warfarin in the treatment of antieoagulation forward. Methods： Among the 66 patients registered,38 were male and 28 were female,the dose of warfarin and the hemorrhage and other adverse effect of warfarin therapy were observed.Results： The dose of warfarin achieved the goal by the INR was different.the keeping dose of warfarin was 1.5 - 5mg,the dose of warfarin in 80.2% patients was 2 - 3.5mg,the INR ranged from 2 to 3,the rate of slight hemorrhage was 7.6%. No one terminated the treatment due to severe hemorrhage, Conclusion： It was far - reaching significance to use warfarin in the patients with atrial fibrillation and the dose of warfarin was difference. And warfarin was safe for the patients with atrial fibrillation.
