摘 要:目的探讨骨水泥强化对正常骨密度和严重骨质疏松两种骨质动力髋螺钉固定稳定性的生物力学影响。方法将48对符合本试验要求的成年股骨上段标本分成正常骨密度组和严重骨质疏松组,每组各24对,均制成股骨转子间骨折,动力髋螺钉固定。每对标本中,右侧骨水泥强化,左侧作为对照。标本进行弯曲强度试验、破坏试验及扭转强度试验。结果弯曲强度试验中正常骨密度组位移减少(0.3959±0.1311)mm,严重骨质疏松组位移减少(2.4134±0.1913)mm;破坏试验中2组负荷升高分别为(149.1935±105.1737)N和(910.5200±174.4108)N;扭转强度试验中标本顺时针方向扭转到8.8Nm,2组转角减少分别为(2.7189±0.1274)°和(4.4568±0.3816)°,3种实验数据显示2组均有统计学意义,P〈0.05。结论骨水泥对骨质疏松骨折稳定性的影响要明显高于正常骨密度的骨折。Objective To evaluate the biomechanical influence of bone cement augmentation to the stability of the dynamic hip screw (DHS) fixation in normal bone density and severe osteoporosis, Methods According to test requirement, forty - eight matched pairs of adult upper femora were used to make the specimen of intertrochanteric fracture . All the specimen were fixed by DHS and divided into two groups , 24 pairs in normalbone density and other for severe osteoporosis, The right one from each pair was augmented with bone cement, whereas the left femur served as controls, In a servohydraulic testing machine, femoral specimens were fixed to perform the biomechanical test which included the bending stiffness, torsional stiffness and load to failure. Results After DHS were augmented by the bone cement, normal bone group displacement reduced (0.3959±0.1311)mm on average , and severe osteoporosis one was (2.4134±0.1913) mm for the bending test. In the failure test, there was (149.1935±105. 1737) N and (910.5200±174.4108 )N of load increasement in two groups. When specimen turned around to 8. 8Nm in anticlockwise direction for torsional test, mean corner decreasement of (2.7189±0.1274)° in normal bone, another was(4.4568±0.3816)°. The statistical result of three groups all showed P 〈0. 05, as a result , there was significant difference between normal bone group and severe osteoporosis one. Conclusion As for stability of fracture, bone cement have more significantly effect on the group of osteoporosis than one of normal bone density.
