肝硬化门脉高压性巨脾行刮吸法原位脾切除术的临床研究

摘　　要：目的探讨刮吸法原位脾切除术的优越性。方法对肝硬化门脉高压性巨脾患者100例采用刮吸法原住脾切除，观察术中出血量、术后腹水情况、术后引流管变清时间、引流液淀粉酶值、术后发热情况、术后门静脉血栓发生率、术后住院天数等指标并与另100例患者采用传统托脾法脾切除术作比较。结果刮吸法原位脾切除术较传统托脾法脾切除术术中出血量少，术后引流管变清时间短，无引流液淀粉酶值增高、术后发热轻、持续时间短，术后门静脉血栓发生率低，术后住院天数短。结论刮吸法原位脾切除术较传统托脾法脾切除术设计合理，此法可减少术中损伤和出血，减少术后并发症的发生。Objective To investigate the advantage of Orthotopic Splenectomy by Curettage and Aspiration Technique for treatment of megalosplenia. Methods To compare the amount of bleeding in surgery, ascitic situation after surgery, drainage clear time, the amylase （AMS） of the draining liquid, height and stage of fervescence, the incidence of portal thrombus and postoperative hospitalization, et al, in megalosplenia which were caused by hepatic cirrhosis and portal hypertension, between the orthotopic curettage and aspiration technique group, and the traditional method group. Results Compared with the traditional method group, there were significant differences between two groups, in terms of the amount of bleeding, the ascitic fluid, drainage clear time, AMS of the draining, postoperative fever, incidence of portal thrombus and postoperative hospitalization in the orthotopic curettage and aspiration technique group. Conclusion Compared with traditional technique, orthotopic splenectomy by curettage and aspiration technique is rational. It can reduce the injury, bleeding and complications which may be caused by traditional surgery.
