高温生理盐水局部灌注兔大脑皮层抑制青霉素诱导癫痫的研究

摘　　要：目的探讨高温生理盐水局部灌注兔大脑皮层能抑制青霉素诱导癫痫发作.方法72只兔3组(n=24):假手术(S)组,高温(65℃)灌注(HP)组,常温(23℃)灌注组(OP).于2 d、3 d、7 d和30d观察各组灌注后毁损部位组织形态病理改变和超微结构改变,评价运动功能障碍.另将72只兔分3组(n=24):假手术(S)组:高温(65℃)灌注(HP)组,未灌注(NHP)组,于0 h、2 h、6 h、24 h观察各组EEG的变化和测定海马组织的GABA,Glu含量,评价高温生理盐水局部灌注抑制癫痫发作的有效性.结果(1)行为学和EEG观察:高温生理盐水局部灌注使癫痫发作平均在2 min内停止,且EEG显示尖波,棘波消失;(2)病理改变:HP组皮层灌注区浅层可见有变性,轻度局部脑水肿,软膜下少量出血,但深部未见异常.电镜观察:浅层可见神经细胞肿胀或固缩,甚至出现胞浆胞核的溶解,树突、线粒体肿胀,轴突髓鞘苍白、偶有脱髓鞘现象.深层结构未见明显异常.OP组和S组未见明显异常;(3)S组海马组织GAGA和Glu含量在各时间段基本无变化.HP组GABA在各时间段逐渐增加,而Glu含量逐渐减少,以2 h改变最为明显,与S组、NHP组比较均P<0.05,有统计学意义.NHP组0 h的GABA含量明显较S组低,而Glu含量却明显高于S组,NHP组在2 h内GABA和Glu含量变化不明显,6 h和24 hGABA有所上升,Glu略有下降.结论高温生理盐水局部灌注可以抑制青霉素诱导的癫痫发作.[Abstract] Objective To investigate the seizure of rabbit induced by penicillin can be inhibited by high temperature normal saline perfusing. Methods Part I of experiment： the rabbits were randomly divided into 3 groups：sham operation group（S）, high temperature normal saline perfuse groups（HP）; ordinary temperature normal saline perfuse groups（OP）. We reviewed the change of pathobiology and ultramicrostructure in the damaging tissue and observed the influence of motor region in 2 hours, 3 days , 7days 30 days. Part Ⅱ of experiment： the rabbits were randomly divided into 3 groups：sham operation group（S）, high temperature normal saline perfuse groups（HP）, none high normal saline perfuse groups（NHP）. We determined γ -aminobutyric acid（GABA）, glutamate（Glu） in hippocampus at 0 h, 2 h, 6 h, 24 h by means of high performance liquid chromatography and observe the changes of EEG. Resuits Seizure can be inhibited by 65℃ normal saline regionally perfusing rabbit cortex in 2 minutes.the damage of the perfusing tissue is ordy in superficial layer. The function of the deep layer was not effected. The level of GABA was heighened postoperation at 0 h , 2 h, 6 h, 24 h, the level of Glu was degraded. The lever in 2 h was significantly changed in hippocampus.there was significantly difference between HP groups and NHP groups（P 〈 0.05）; HP groups and S groups（P 〈 0.05）. Conclusion The seizure induced by penicillin can be inhibited by 65℃ normal saline regionally perfusing cortex.
