男方HBV慢性感染对体外受精-胚胎移植治疗结局的影响

摘　　要：目的探讨男方乙型肝炎病毒(HBV)慢性感染对体外受精-胚胎移植(IVF-ET)治疗结局的影响。方法以男方HBsAg阳性而女方HBsAg阴性的不孕症夫妻102对为研究组;双方均HBsAg阴性的不孕症夫妻204对为对照组,比较两组的累积胚胎评分(CES)、受精率、卵裂率、优质胚胎率、种植率、临床妊娠率、早期流产率、晚期流产率、分娩率和新生儿畸形率。结果102例男方HBsAg阳性组与204例男方HBsAg阴性组的累积胚胎评分(52.8±18.7对55.4±16.9)、受精率(66.9%对66.1%)、卵裂率(97.6%对97.2%)、优质胚胎率(34.0%对37.1%)、种植率(40.9%对34.6%)、临床妊娠率(56.9%对50%)、早期流产率(6.9%对5.9%)、晚期流产率(8.6%对4.9%)、分娩率(40.2%对43.6%)、新生儿畸形率(0对0),两组间无统计学差异(P>0.05)。结论男方慢性HBV感染不影响IVF-ET治疗的结局。Objective To investigate the outcome of in vitro fertilization and embryo transfer （IVF-ET） in couples with the husband positive for chronic infection of hepatitis B virus （HBV）. Methods This study involved 102 infertile couples receiving IVF-ET with the husbands （but not the wives） positive for hepatitis B surface antigen （HBsAg）, and another 204 couples negative for HBsAg receiving the treatment served as the control group. The cumulative embryo score, fertilization rate, cleavage rate, rate of good quality embryos, implantation rate, clinical pregnancy rate, first trimester and late miscarriage rates, delivery rate, and neonatal malformation rate were recorded and compared between the two groups. Results Between the HBsAg-positive and the control groups, the cumulative embryo score （52.8±18.7 vs 55.4±16.9）, insemination rate （66.9% vs 66.1%）, cleavage rate （97.6%vs 97.2%）, rate ofgoodqualityembryos （34.0%vs 37.1%）, implantation rate （40.9%vs 34.6%）, clinical pregnancy rate （56.9% vs 50%）, first trimester miscarriage rate （6.9% vs 5.9%） and late pregnancy miscarriage rate （8.6% vs 4.9%）, delivery rate （40.2% vs 43.6%） and neonatal malformation rate （0 vs 0） were all similar （P〉 0.05）. Conclusion Chronic HBV infection in the husband might not affect the outcome of IVF-ET treatment.
