核素肾动态显像评价2型糖尿病早期肾功能

摘　　要：目的探讨核素肾动态显像在评价2型糖尿病肾病早期肾功能损害中的应用价值。方法2型糖尿病患者68例,按照尿白蛋白含量的不同分为3组:正常白蛋白尿组(DM1)、微量白蛋白尿组(DM2)、临床白蛋白尿组(DM3),遴选健康人12名作为对照组(NC)。各组分别行^(99m)Tc-DTPA肾动态显像及实验室UAER、Scr、尿β2-MG的测定。结果DM1组GFR较NC组有显著性升高(P<0.01),DM3组GFR较NC组显著降低(P<0.01),DM2组GFR与NC组无差异;DM2组肾功能参数T_(1/2)、C_(20)值较NC组明显增高(P<0.05),DM3组肾功能参数T_P、T_(1/2)、C_(20)值较NC组显著增高(P<0.01),DM1组肾功能各项参数与NC组无差异。结论核素肾动态显像可早于UAER、β_2-MG、Cr等实验室检测指标对2型DN作出准确诊断,可全面评价2型DN的肾功能损害情况。Objective To evaluate the renal dynamic imaging in the judgment of the renal function in patients with type -2 diabetes mellitus so as to provide reference for clinical treatment and prognosis predicting. Methods 68 patients with type 2 diabetes mellitus were allocated to a DM1 group （n = 12, UAER 〈 20μg/min） and a DM2 group （n =26, UAER 〈 20 μg/min -200μg/ min） and DM3 group （n =30, UAER UAER 〉 200μg/min） according to their UAER. The comparison of the glomerular filtration rate （GFR） , the renal function curve peaktime （Tp） and the 20 -min residual rate （C20） and drainage half time （T1/2 ） parameters to the control group （ NC, n = 12） had also been done. Results In the DM1 group, the GFR was statistically higher than that in control group （ P 〈 0.01 ）, and the GFR in the DM3 group was statistically lower than that in control group （ P 〈 0. 01 ）. The renal function parameters of T1/2 and C20 in the DM2 and DM3 groups much more prolonged than that in the control group （ P 〈 0.05 in DM2/NC and P 〈 0. 01 in DM3/NC） , especially the Tp in the DM3 group much more prolonged than that in the control group too （ P 〈 0.01 ） . Conclusions The renal dynamic imaging can diagnose the GFR in type - 2 diabetes mellitus earlier than the laboratory results, judge the injury of the renal function and provide reference for clinical treatment and its follow -up.
