微波热凝联合慢咽合剂治疗慢性肥厚性咽炎疗效观察及对咽部病损组织PCNA表达的影响

摘　　要：目的:观察微波热凝联合慢咽合剂治疗慢性肥厚性咽炎的临床疗效及对咽部病损组织增殖细胞核抗原(PCNA)表达的影响。方法:将70例慢性肥厚性咽炎患者,随机分为2组各35例。微波热凝加慢咽合剂组(简称治疗组),采用微渡热凝治疗后,加服自拟的慢咽合剂治疗;单纯微波热凝组(简称对照组),仅采用微波热凝治疗。观察2组临床疗效及症状改善情况;2组分别在治疗前及治疗结束后2周作PCNA测定。结果:2组临床疗效比较,治疗组明显优于对照组(P<0.05);治疗组在消除咽异物感及咽部淋巴滤泡增生方面明显优于对照组(P<0.05);2组治疗后PCNA表达阳性率均明显低于治疗前,差异有非常显著性意义(P<0.01),治疗组治疗后PCNA表达亦明显低于对照组,差异有非常显著性意义(P<0.01)。结论:微波热凝联合慢咽合剂治疗慢性肥厚性咽炎可抑制咽部病损组织的细胞增殖特性,从而降低咽部病损组织的PCNA表达,临床疗效显著。Objective:To observe the therapeutic effect of micro-wave thermo- coagulation combined with Manyan Mixture(MM)for chronic hypertrophic pharyngitis and its effect on proliferating cell nuclear antigen(PCNA)expression in affécted pha- ryngeal tissue.Methods:Seventy patients were equally randomized into 2 groups, The two groups received micro-wave thermocoagulation,and the treatment group re- ceived oral use of MM additionally.The therapeutic effect was evaluated after treat- ment,and the PCNA expression in affected pharyngeal tissue before treatment and 2 weeks after treatment was also detected.Results:The therapeutic effect in the treat- mere group was better than that in the control group(P<0.05).The treatment group had a better effect on relieving foreign body sensation in pharynx and the proliferation of pharyngeal lymph follicle than the control group(P<0.05).The positive rate of PCNA expression in the two groups decreased after treatment(P<0.01)as compared with those before treatment),and was lower in the treatment group than that in the control group(P<0.01).Conclusion:Micro-wave thermocoagulation combined with MM has a better effect for chronic hypertrophic pharyngitis by inhibiting the cell proliferation of affected pharyngeal tissue and reducing PCNA expression.
