臀肌挛缩症的手术治疗

摘　　要：目的探讨臀肌挛缩症的手术治疗的疗效。方法臀肌挛缩症45例90侧臀，行单纯臀大肌挛缩带切断松解，保留正常臀大肌62侧臀；臀大肌挛缩带“Z”形延长28侧臀。其中行臀中肌、臀小肌部分切断松解15侧臀，行髂胫束“Z”形延长48侧臀，行皮下组织切开、皮肤松解12侧臀，均不缝合深筋膜。结果全部随访，时间3-41个月，平均21个月，均术后功能改善满意，轻度摇摆步态2例（4．44％）。结论臀肌挛缩症采用臀肌挛缩带松解延长或结合髂胫束延长等，手术操作简便，功能恢复快，疗效满意。Objective To investigate the therapeutic effect of operation for gluteal muscle contracture. Methods 62 cases were performed with cutting contracture fascia lata,28 cases of whom were performed with Z- shaped lengthening. 15 with releasing glutens medius muscle or glutens minimus muscle. Tractus iliotibialis were performed with Z-shaped lengther6ng in 48 cases. 12 cases were performed with releasing skin and subcutaneous tissue. Results 45 patients were followed up from 3 to 41 months on an average of 21 months. All patients were satisfactory with their postoperative improvements. 2 patients { 4.44% ） remained mild swinging gait. Condusion Combined therapy of cutting and lengthening contracture fascia lata and lengthening tractus iliotibialis may gain satisfactory effects for patient with gluteal muscles contracture. The operation is easy, safe and effective.
