小儿肺炎支原体脑炎近期预后的预警因素分析

摘　　要：目的探讨影响小儿肺炎支原体（MP）脑炎近期预后的预警因素。方法对59例MP脑炎患儿的临床及实验室资料进行回顾性分析．对近期预后进行评估，寻找可能的预警因素。结果与近期预后良好组相比，近期预后不良组的MP脑炎患儿意识障碍、惊厥及发热持续的时间均较长．多伴有肢体运动障碍，而且除脑实质损害外，伴有其他脏器功能损害（OR：11．36，P〈0．01）、脑电图表现为弥漫性慢波或阵发性痫样放电伴低电压（OR：25．81．P〈0．01）及头颅影像学异常（OR：8．75，P〈0．01），其在多因素Logistic回归分析中有显著意义。结论MP脑炎患儿近期预后不良的预警因素是患儿伴有其他脏器功能损害、脑电图表现为弥漫性慢波或阵发性痫样放电伴低电压和头颅影像学异常。Objective To explore the predictive factors of short-term prognosis in children with mycoplasrrla pneumonia （MP） encephalitis. Methods The clinical and experimental data of 59 cases with MP encephalitis were retrospectively analyzed and short-term outcomes were evaluated, in order to find the possible predictive factors. Results Durations of the consciousness disturbance, seizures and fever of children with MP encephalitis in the poor short-term prognosis group were comparatively longer, frequently accompanied by motor disorders of limbs and functional damage to other organs as well as diffuse slow waves or paroxysmal epileptiform discharges with low voltage in electroencephalogram （EEG） and abnormal cranial imaging, compared with that of children with MP encephalitis in the favorable short-term prognosis group; differences between the two groups were significant （P〈0.05）. Except for cerebral parenchymal impairment, the accompanying functional damage to other organs （OR： 11.36, P〈0.01）, diffuse slow waves or paroxysmal epileptiform discharges with low voltage in EEG （OR： 25.81, P〈0.01） and abnormal cranial imaging （OR： 8.75, P〈0.01） were all predictive factors of poor short-term outcomes of MP encephalitis. Conclusion Predictive factors of poor short-term prognosis of children with MP encephalitis are the accompanying functional damage to other organs, diffuse slow waves or paroxysmal epileptiform discharges with low voltage in EEG and abnormal cranial imaging.
