射频热凝术损毁腰交感神经节治疗下肢缺血性疾病患者的护理和健康指导

摘　　要：目的探讨射频热凝术（radio frequency ablation，RFA）损毁腰交感神经节治疗下肢缺血性疾病的术后护理，结合各项指标的监测，为治疗提供依据。方法对40例RFA腰交感神经节治疗下肢缺血性疾病患者实施心理护理术后并发症的观察，患肢趾尖血氧饱和度（SpO2）、趾脉搏波幅、足背皮肤温度等指标的监测，及时采取相应的措施和健康宣教。结果40例患者无一例出现内脏损伤，患趾脉搏波幅较术前升高（6．0±0．2）mm，患肢足背皮温较术前升高（1．2±0．2）℃，术后溃疡病灶好转率占40％，痊愈率占15％。结论通过对患肢血氧饱和度、足趾脉搏波幅及疼痛的评分，足背皮肤温度的监控，为术后康复治疗提供有效的依据；积极处理溃疡病灶，适时地对患者进行健康宣教，减少了并发症的发生，缩短了住院时间，使患者达到身心的全面康复。Objective To discuss the postoperative nursing after Radio FrequencyAblation（RFA） to destruct waist sympatheric ganglia to treat ischemic disease of lower extremity and supply reference for treatment with the combination of a series of monitoring indicators. Methods We gave mental nursing, monitoring of postoperative complication,SpO2, toe pulse and skin temperature of dorsal foot to patients and adopted corresponding measures and health education. Results No visceral injury was observed in 40 cases. The wave amplitude of toe pulse increased by （6.0±0.2）mm and skin temperature to （1.2±0.2）℃ in the affected limb compared with preoperation conditions. The improvement rate of ulcer is 40% and the cure rate is 15%. Conclusion Close monitoring of the above mentioned parameters can supply effective references for postoperative rehabilitation. Positive handling of ulcer and appropriate health education can reduce complications and hospitalization duration and ensure comprehensive rehabilitation of patients.
