摘　　要：目的：探讨和总结成人股骨头缺血性坏死的治疗方案。方法：成人股骨头缺血性坏死45例51髋，均行头颈开窗、死骨清除、带血供骨瓣并松质骨髓植入术。结果：45例患者经过1.5-6年的随访，以广州中医药大学附属医院提出的疗效标准（百分法）评价治疗效果，优18髋、良23髋、可5髋、差5髋，优良率80.4％，总有效率90.2％。结论：带肌蒂骨瓣并松质骨髓移植，彻底清除死骨和减压，可恢复股骨头血运及股骨头外形，适宜于FicatⅠ～Ⅲ期青壮年股骨头缺血性坏死的患者。Objective：To explore and summarize the treatment plan for the adult ischemic necrosis of femoral head.Methods：51 hips in 45 patients were treated by fenestration operation on the neck of the femoral head,scavenglng sequestra,implanting blood supplying bone flap and spony bone marrow. Results：All the patients were followed up for 1.5-6 years.According to evaluating standard of curative effect set by the Affiliated Hospital of Guangzhou University of Traditional Chinese medicine,the excellent result was achieved in 18 hips,good in 23 hips,fair in 5 hips and poor in 5 hips. The excellent and good rate was 80.4%,and effective rate was 90.2%. Conclusion：The myo-peduncle bone flap and the spony bone marrow transplant can clearing sequestra completely,decompress and recover blood supply and the shape of femoral head.It′s effective in the treatment of the adolescent and postadolescent patients with ischemic necrosis of femoral head at Ficat Staging Ⅰ-Ⅲ.
