婴幼儿先天性巨结肠术前灌肠效果的Meta分析

摘　　要：目的评价婴幼儿先天性巨结肠术前采用改良灌肠法的灌肠效果。方法通过数据库检索国内相关文献,并按Cochrane协作网提供的Meta分析方法对其进行定量综合分析,比较两组不同灌肠方法的效果。结果8篇文献共495例病人符合纳入标准并进入研究。其中住院时间指标存在明显异质性(χ2=470.54,P<0.01),不进行简单合并分析;术前灌肠时间指标存在异质性(χ2=57.56,P<0.01),采用随机效应模型分析,结果显示观察组的术前灌肠时间明显少于对照组(P<0.01);肠腔清洁度以及术后并发症两项指标均不存在异质性,采用固定效应模型分析,说明改良灌肠法的肠腔清洁度优良率明显高于传统灌肠法,而术后并发症发生率则明显减少(P<0.01)。结论改良后的灌肠方法提高了灌肠效果,但更加系统性的随机对照试验结果的支持是必需的。Objective To evaluate the therapeutic effect of improved clysis in infants with congenital megacolon before operation. Methods Meta-analysis provided by Cochrane operation network was used for quantitative aggregate analysis for related articles searched from CBMdisc, VIP and google. The therapeutic effects were compared between the observation group and the control group. Results Four hundred and ninty-five patients from 8 references met to internalizing standard. The index of the hospitalized time was obvious heterogeneity （χ^2=470.54, P〈0.01 ）, which could not be simply aggregate analyzed. The index of the preoperative clysis time was general heterogeneity （χ^2=-57.56,P〈0.01）,analysis was done with random effects model （P〈0.01）.The result illustrated that the preoperative clysis time in the observation group was obviously shorter than that of the control group. Both the index of clean degree of intestine and the rate of postoperative complication had no heterogeneity, analysis was done with fixed effects model （P〈0.01）.The result illustrated that the clean degree of intestine of the improved clysis obviously raised and the rate of postoperative complication obviously decreased. Conclusion The improved clysis could rais the efficiency of clysis and could been applied for clinical practice. However, the results with more and well designed randomized controlled trials are necessary.
