摘  要:目的:研究自制皮质骨锚钉修复肩关节不稳的生物力学性能。方法:采用新鲜成年猪胫骨皮质骨,制成剑形皮质骨锚钉3.5mm×7.0mm;孔内穿入2号Ethibond不可吸收缝线备用。实验分2组,采用FastinRC金属螺纹锚钉作为对照组,将剑形皮质骨锚钉和金属螺纹锚钉各1枚分别植入同一个猪肱骨头与大结节之间处,将肱骨干使用夹具固定在工作平台上,另一端缝线悬吊于生物力学测试机上,进行生物力学垂直拔出试验。结果:皮质骨锚钉10枚其中7枚拔出,1枚缝线拔断,2枚锚钉损坏。最大拔出载荷平均为181.2N。FastinRC金属螺纹锚钉组8枚锚钉拔出,2枚缝线断裂,锚钉无损坏,平均最大拔出载荷为163.8N。两者差异不显著。皮质骨锚钉抗压测试结果为(890.0±124.3)N。结论:皮质骨锚钉生物力学性能达到修复肩袖损伤初始固定强度的要求。Objective:To evaluate the biomechanical features of a self-made cortical bone anchor for shoulder instability repair. Methods:Fresh porcine fresh tibial cortical bone was made to ancipital cortical anchors 3.5mm × 7.0mm; Ethibond NO. 2 non-absorbable suture was pulled through the eyelet for preparation. All specimens were divided to two groups, in control group the FastinRC metal screw was used. An ancipital cortical anchor and a metal screw were embeded into the area between humerus head and greater tubercle separately in the same specimen. The shaft of humerus was fixed on the work desk using clamp, and the end of suture was hanged on the biomechanical testing machine, the biomechanical pull-out testing was performed. Results:The failure modes in cortical bone anchor were eyelet failure in 2 cases, suture failure in 1 case, and anchor pullout in 7 cases. The mean failure load was 181.2N. In FastinRC metal screw group the failure modes were suture failure in 2 cases, and anchor pullout in 8 cases. There was no eyelet failure, the mean failure load was 163.8N. There was no statistical difference between the two groups. The mean data of press test in cortical bone anchor was 890.0 ± 124.3N. Conclusion :The biomechanical property of cortical bone anchor reaches the need of initial capability for shoulder instability repairing.
