益气补肾活血法防治家兔膝骨性关节炎的实验研究

摘　　要：目的:观察益气补肾活血法对家兔膝骨关节炎(Osteoarthritis,OA)模型的疗效及可能的作用机制。方法:将60只新西兰大耳兔随机分成4组,用制动法复制OA模型。造模1周后,A、B组用生理盐水,C组用壮骨关节丸,D组用益气补肾行血方灌胃。于灌胃后第7、13周,对组织病理学、软骨细胞增殖和凋亡进行观察。结果:第7、13周,D组病理总积分明显低于B、C组(P<0.01)。第7周,在增殖指数上,D组明显高于C组(P=0.000);在凋亡指数上,C、D组低于B组(P<0.01),C、D组间无明显差异(P=0.567)。第13周,在增殖指数上,B、C、D各组之间均无明显差异(P>0.05);在凋亡指数上,D组明显低于C组(P=0.025)。结论:益气补肾活血法可以明显改善实验家兔OA病理积分,并能有效抑制关节软骨细胞过度凋亡和促进其增殖。推测其作用机制,在OA的初中期,通过促进软骨细胞增殖和抑制其凋亡同时起作用;而在中晚期主要靠抑制其过度凋亡起作用。Objective： To investigate the effects and possible mechanisms of a new herbal recipe, yiqibushenxingxue recipe （YQBSXX） on osteoarthritis （OA） in the knee joint in rabbit models. Methods ： 60 New Zealand rabbits were randomly divided into 4 groups. All animals in group B - D were managed by immobilization. One week later, group D was treated with YQBSXX recipe, group C with Zhuanggugnanjie recipe, groups A - B were received normal saline. The gross morphological condition, the proliferation and apoptosis of ehondrocytes were observed in the 7^th week and 13^th week respectively. Results： In the 7^th week and 13^th week after consumption, according to C. Colombo score criterion, the average score in group D was lower than that of group B - C （ P 〈 0.01 ）. In the 7 ^th week, the ehondrocytes proliferative index in group D was obviously higher group C （ P = 0.000） ; the ehondroeytes apoptosis index in group C-D were obviously lower than group B（ P 〈0.01 ）, but there was no obvious difference between group D and group C（ P =0.567）. In the 13^th week, there was no obvious difference in the ehondrocytes proliferative index in group B - D, but the ehondrocytes apoptos index in groups D was obviously lower than that of group C （ P = 0. 025 ）. Results： The treatment of OA in a rabbit model with YQBSXX recipe shows significantly better histologic results , it can restrain effectively chondrocytes apoptosis and promote chondrocytes proliferation. To the mechanisms of the treatment, both restraining chondrocytes apoptosis and promoting proliferation probably have great meanings in the early / middle stages, but restraining ehondrocytes apoptosis probably plays a main role in middle/late stage.
